平阳霉素碘化油乳剂动脉栓塞治疗巨大肝血管瘤疗效及安全性评价

The efficacy and safety evaluation of pingyangmycin lipiodol emulsion in the hepatic artery embolization treatment of hepatic hemangioma

目的:探讨采用平阳霉素碘化油乳剂进行肝动脉栓塞术治疗巨大肝血管瘤的疗效和安全性。方法:对21例巨大肝血管瘤患者,采用平阳霉素碘化油混合乳剂超选择血管瘤供血动脉插管栓塞治疗,观察治疗前后3、6个月时患者临床症状、肿瘤大小变化以及并发症情况。结果:在术后三个月以及六个月时,所有患者进行复查,给予CT平扫与MR检查,瘤体血供完全消失或者显著减少,其内血供消失,碘油沉积良好,治疗前有临床症状者治疗后缓解或消失。手术之后患者没有出现肝坏死、胆管狭窄以及胆囊坏死等多种严重并发症。结论:对于治疗巨大肝血管瘤患者而言,采用平阳霉素碘化油乳剂相对安全,且效果良好,值得推广应用。

Objective To investigate the efficacy and safety of pingyangmycin iodized oil emulsion in the treatment of giant hepatic hemangioma with hepatic artery embolization. Methods Twenty-one patients with giant hepatic hemangioma were treated with pingyangmycin iodized oil mixed emulsion for superficial selection of hemangioma with intubation and embolization. The clinical symptoms, tumor size changes and complications were observed at 3 and 6 months before and after treatment.Results All patients underwent re-examination at three months and six months after surgery. CT scan and MR examination were performed. The blood supply of the tumor completely disappeared or decreased significantly. The blood supply disappeared and the oil of iodized oil was well deposited. Clinical symptoms are relieved or disappeared after treatment. There were no serious complications such as hepatic necrosis, bile duct stricture and gallbladder necrosis after surgery. Conclusion Pingyangmycin iodized oil emulsion for hepatic artery embolization is a safe and effective minimally invasive treatment for giant hepatic hemangioma. It is worthy of popularization and application.
