摘要
目的:探究在胎儿先天性心脏病的诊断中,彩色多普勒超声心动图的临床应用效果。方法:对我院妇产科在2017年6月—2018年12月收录的3687例孕妇进行胎儿先天性心脏病的产前诊断,采用彩色多普勒超声心动图诊断方法。将所有产后胎儿进行进一步的超声检查及病理分析,同时做一定时期的随行访问调查,得出彩色多普勒超声心动图诊断的准确率及漏诊率。结果:在胎儿出生后进行准确诊断,得出共有53例(1.44%)患儿患有先天性心脏病症状,其中大血管错位的有3例(5.66%),左动脉缩窄的有8例(15.09%),房间隔发生缺损的有16例(30.19%),动脉导管未闭的有22例(41.51%),复杂先心病的有4例(7.55%)。经彩色多普勒超声心动图诊断确诊为阳性的有48例(1.31%),准确率高达90.57%,漏诊率为9.43%。结论:在胎儿先天性心脏病的临床诊断治疗中,彩色多普勒超声心动图诊断方法具有较高的精确判断度,具有较高的临床应用价值。
Objective To explore the clinical application of color Doppler echocardiography in the diagnosis of fetal congenital heart disease.Methods The prenatal diagnosis of fetal congenital heart disease was performed in 3687 pregnant women from June 2017 to December 2018 in the Department of Obstetrics and Gynecology of our hospital.The diagnosis method was color Doppler echocardiography.All postpartum fetuses were further examined by ultrasonography and pathological analysis.At the same time, follow-up visits were made for a certain period of time to obtain the accuracy and missed diagnosis rate of color Doppler echocardiography.Results A total of 53 cases(1.44%) suffered from congenital heart disease after the birth of the fetus. Among them, 3 cases(5.66%) suffered from macrovascular malposition, 8 cases(15.09%)suffered from left artery constriction,16 cases(30.19%) suffered from atrial septal defect,22 cases(41.51%) suffered from patent ductus arteriosus.There were 4 cases of heart disease(7.55%).48 cases(1.31%) were diagnosed as positive by color Doppler echocardiography.The accuracy rate was as high as 90.57%,and the missed diagnosis rate was 9.43%.Conclusion In the clinical diagnosis and treatment of fetal congenital heart disease,the diagnostic method of color Doppler echocardiography has high accuracy and clinical value.
