摘要
目的:探讨动态对比增强磁共振成像(DCE-MRI)技术对子宫肌瘤的诊断价值。方法:收集2014年1月—2016年10月间于我院妇科住院并行DCE-MRI检查的80例疑似子宫肌瘤患者资料,所有患者均行手术治疗,DCE-MRI诊断结果以手术病理诊断为参照。其中,将手术病理确诊为子宫肌瘤的67例患者设为子宫肌瘤组,另招募60例健康妇女作为对照组,所有被试者DCE-MRI技术检查后采用4D-Tissu软件自动分析,通过生成信号强度-时间曲线,比较两组的DCE-MRI扫描参数。结果:DCE-MRI对子宫肌瘤的诊断灵敏度、特异度、准确性分别为97.01%、92.31%、96.25%,与手术活检病理诊断结果一致性较高;另外,与对照组比较,子宫肌瘤组的峰值时间明显缩短、表观扩散系数明显降低、最大信号强度明显增强,组间比较差异均具有统计学意义(P <0.05)。结论:DCE-MRI技术对子宫肌瘤的诊断准确性高,为临床诊断子宫肌瘤提供无创可靠的影像学方法。
Objective To evaluate the diagnostic value of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)technique in uterine leiomyoma.Methods 80 suspected hysteromyoma patients were recruited in our hospital from January 2014 to October 2016.According to the results of biopsy and pathology,the patients which were identified with uterine leiomyoma were defined as uterine leiomyoma group,and 60 healthy women were defined as control group.All the subjects were scanned by DCE-MRI technique.The time-signal intensity curve was analyzed automatically by 4D-Tissu software,and the parameters of DCE-MRI scanning were compared between the two groups.Results The sensitivity and specificity of dynamic enhanced scan for uterine leiomyoma were observed.The accuracy rate was 97.01%92.31%and 96.25%,respectively,which was in good agreement with the pathological diagnosis of surgical biopsy.In terms of DCE-MRI scan parameters,it was significantly higher than that in the control group.In uterine leiomyoma group,the peak time was shortened significantly(P<0.05),the apparent diffusion coefficient was significantly decreased(P<0.05),and the maximum signal intensity was significantly enhanced(P<0.05).Conclusion DCE-MRI technique has high accuracy in the diagnosis of uterine leiomyoma,it may provide a noninvasive and reliable imaging method for clinical diagnosis of uterine leiomyoma.
