临产期UTA及胎儿MCA与新生儿Apgar评分相关性分析

摘要 目的:研究临产期孕妇的UTA及胎儿MCA的血流参数变化与新生儿Apgar不同评分之间的相关性,探讨临产期UTA与胎儿MCA联合诊断不同分级胎儿宫内窘迫的应用价值。方法:对151例临产孕妇UTA及胎儿MCA的PI、RI、S/D等血流参数常规进行测量,产后对新生儿进行Apgar评分。评分标准为10~8分的为正常组,7~5分的为轻度窒息组,≤4分以下的为重度窒息组。以正常组为对照组,以轻度窒息组和重度窒息组为观察组,对比各组UTA及胎儿MCA的PI、RI、S/D的血流指标变化并进行分析。结果:临产期胎儿MCA/Ut A-PI、MCA/Ut A-RI、MCA/Ut A-S/D测值随着新生儿评分的降低逐渐减低,以轻度窒息组MCA/UtA-PI减低最为显著,与正常组对照显著差异(P <0.05)。其中MCA/UtAPI <1.55、MCA/Ut A-RI <1.39、MCA/Ut A-S/D <1.61可以作为诊断轻度窒息的临界值,结论:UTA和MCA联合对早期诊断临产期胎儿宫内窒息具有较高的价值,但因重度窒息组收集到的数据太少,需要在以后的研究中继续对照分析。
