经阴道彩色多普勒超声在剖宫产疤痕妊娠诊断中的价值及体会

The value and experience of transvaginal color Doppler ultrasound in the diagnosis of cesarean scar pregnancy

目的:分析探讨经阴道彩色多普勒超声在剖宫产疤痕妊娠诊断中的价值及体会。方法:从我院2015年1月至2019年1月经超声诊断的剖宫产疤痕妊娠患者中选择36例作为研究对象,对比分析经腹部和经阴道彩色多普勒超声检查诊断的准确性。结果:经阴道彩色多普勒超声检查的诊断符合率为94.44%,而经腹部超声检查为66.67%,明显前者的诊断价值更高,差异显著、具有统计学意义(P <0.05)。结论:经阴道彩色多普勒超声具有较高的临床诊断价值,能够清晰显示孕囊、包块与疤痕的关系,并能够显示包块或孕囊的血供来源,是早期疤痕妊娠的检查手段。但诊断时需结合患者既往史、血HCG检查值等临床资料综合分析,以防误诊。

Objective To analyze the value and experience of transvaginal color Doppler ultrasound in the diagnosis of cesarean scar pregnancy.Methods From January 2015 to January 2019,36 cases of cesarean scar pregnancy diagnosed by ultrasound in our hospital were selected as the research object,and the diagnostic accuracy of transabdominal and transvaginal color Doppler ultrasound was compared and analyzed.Results The diagnostic coincidence rate of transvaginal color Doppler ultrasound was 94.44%,while that of transabdominal ultrasound was 66.67%.The diagnostic value of transvaginal color Doppler ultrasound was significantly higher than that of transabdominal color Doppler ultrasound(P<0.05).Conclusion Transvaginal color Doppler ultrasound has high clinical diagnostic value,can clearly show the relationship between gestational sac,mass and scar,and can show the blood supply of mass or gestational sac.It is a means of early scar pregnancy detection.However,in order to prevent misdiagnosis,a comprehensive analysis of clinical data such as patient’s past history and blood HCG examination value should be made.
