DKI在胶质瘤分级中的诊断应用 被引量:1

摘要 目的:评估扩散峰度成像(diffusion kurtosis imaging,DKI)在高、低级别胶质瘤中的诊断应用价值。方法:共招募30例疑似胶质瘤患者,在术前行DKI磁共振成像,测量瘤体区及对侧正常脑白质的峰度参数,包括轴向峰度(axial kurtosis,KA)、径向峰度(radial kurtosis,KR)、平均峰度(mean kurtosis,MK);扩散参数,包括轴向扩散率(axial diffusivity,DA)、径向扩散率(radial diffusivity,DR)和平均扩散率(mean diffusivity,MD);各向异性分数(fractional anisotropy,FA),同时标准化得出相应相对值。对比病理,评估胶质瘤分级间参数的不同性,同时应用ROC曲线评估诊断效能。结果:DKI扩散参数扩散参数、FA及其相对值差别不具有统计学意义(P> 0.05);DKI峰度参数及相对值均具有统计学意义(P <0.05),且胶质瘤级别越高,峰度值越高。ROC曲线分析中,MK及其相对值r MK诊断效能最高(AUC=0.813、0.847)。结论:DKI峰度参数在术前胶质瘤分级中的评估有重要诊断价值。
