邯郸地区10306例妇科门诊女性患者HPV基因分型及感染特征分析

HPV Genotyping and Infection Characteristics of 10306 Female Patients in Handan Area

目的了解邯郸地区人乳头瘤病毒(HPV)感染情况,为当地妇女的宫颈癌防治提供依据。方法收集2016年3月至2017年11月到邯郸市中心医院妇科门诊就诊以及进行健康体检的10 306例女性患者为研究对象,采用聚合酶链反应-反向点杂交法对患者的宫颈脱落细胞标本进行23种HPV基因亚型检测。结果 10 306例女性患者HPV总感染率为36. 26%; HPV高危感染的基因亚型前三位为HPV16、HPV52和HPV58; HPV低危感染的基因亚型前三位为HPV42、HPV81、HPV6;五个年龄组中36～45年龄组患者HPV感染阳性率最高为53. 33%,其次为15～25岁年龄组患者,各年龄段HPV感染率比较差异有统计学意义(P <0. 01);各年龄段感染情况多为单一或两重感染,也可见三重、四重、五重及以上的多重感染;其中单一感染率以46～55岁年龄段最高,多重感染率以15～25岁最高。结论邯郸地区女性人群中HPV感染率较高,36～45岁年龄段感染率最高,以单一型高危型感染为主,最常见感染亚型为HPV16、HPV52和HPV58。

Objective To understand human papilloma virus( HPV) infection in Handan area and provide evidence for cervical cancer prevention for local women. Methods From Mar. 2016 to Nov. 2017,10 306 female patients who went to the gynecology clinic of Handan Central Hospital for medical examination were included in the study,and 23 HPV genotypes were detected by polymerase chain reaction-reverse dot hybridization. Results The total HPV infection rate was 36. 26%in 10 306 women. The top three genotypes of high-risk HPV infection were HPV16,HPV52 and HPV58. The top three genotypes of HPV low-risk infection were HPV42,HPV81 and HPV6. Among five age groups,in the 36-45 years age group HPV infection positive rate was the highest( 53. 33%),followed by 15-25 years age group,the difference of HPV infection rate of different age groups was statistically significant( P < 0. 01). The infection in each age group was mostly single or double infection,and triple,quadruple,five types or more multiple infections were also observed;the single infection rate was highest in the 46-55 year age group,while multiple infection rate was the highest in the women from 15 to 25 years old.Conclusion The infection rate of HPV is high among the female population in Handan area;the infection rate is the highest in the age group of 36 to 45 years;the single high-risk type infection is the major type;the most common subtypes are HPV16,HPV52 and HPV58.
