超声引导经皮肾盂造瘘治疗恶性肿瘤所致梗阻性肾功能衰竭

Ultrasound-Guided Percutaneous Pyelostomy in Treatment of Malignant Obstructive Renal Failure

目的:探讨超声引导经皮肾盂造瘘治疗恶性肿瘤所致梗阻性肾功能衰竭的临床价值。方法:对2012年6月至2015年5月中山大学附属东华医院29例因恶性肿瘤导致急性梗阻性肾功能衰竭的患者予以超声引导下经皮肾盂造瘘术,观察术后疗效和并发症情况。结果:29例患者均一次性穿刺置管成功,未见明显严重并发症,术后2~7 d肾功能均有不同程度恢复,其中28例患者肾功能明显改善,1例患者肌酐及尿素氮下降不明显。结论:超声引导经皮肾盂造瘘术安全可靠,操作简单,成功率高,能有效地改善恶性肿瘤所致的肾脏功能衰竭,为延长患者生命或进行下一步治疗提供了保障,具有较高的临床实用价值。

Objective: To investigate the clinical value of ultrasound-guided percutaneous pyelostomy in the treatment of malignant obstructive renal failure. Methods: 29 cases of acute obstructive renal failure caused by malignant tumor accepted ultrasound-guided percutaneous pyelostomy,and the postoperative efficacy and complications were observed. Results: 29 patients were one-time successfully punctured and put indwelling catheter without serious complications. The renal functions recovered to some extent after operation. The renal functions of 28 patients were improved obviously except 1 case. Conclusion: Ultrasoundguided percutaneous pyelostomy is safe,reliable,simple with high success rate. It can effectively improve the renal failure caused by malignant tumor,and provide guarantee for life extension or the next treatment,and has high clinical practical value.
