我院开展“合理用药月”活动对用药监管的成效分析

Analysis of the achievements of the administration of drugs after the implementation of the Rational Use of Drug Month in our hospital

目的探讨解放军总医院海南分院(以下简称'我院')开展'合理用药月'活动对用药监管所取得的成效。方法我院于2014年7月开展'合理用药月'活动。通过收集活动前(2013年7月~2014年6月)和活动后(2014年8月~2015年7月)用药相关数据,从药费比、处方医嘱合格率、抗菌药物和辅助用药使用、医保用药等多个角度,回顾性分析活动前后相关用药指标的变化差异。结果通过开展'合理用药月'活动,我院药费比合理控制在37%左右,门、急诊和住院处方/医嘱合格率分别提升至94.21%、98.79%和98.89%;门、急诊和住院抗菌药物使用率分别控制在6.96%、38.40%和49.65%;Ⅰ类切口抗菌药物预防使用率降至28.61%;抗菌药物使用强度下降了26.74%;总体、限制使用级和特殊使用级抗菌药物送检率分别提升至41.74%、53.86%和72.79%,辅助用药消耗金额构成比下降了17.25%;基本杜绝了医保拒付情况的发生。结论通过开展'合理用药月'活动,我院临床用药更加合理,并为下一步合理用药监管工作提供了思路。

Objective To discuss the achievements of standardizing the use of drug after the implementation of the Rational Use of Drug Month(RUDM) in Hainan Branch of PLA General Hospital('our hospital' for short). Methods The RUDM had been implemented in our hospital in July 2014. The data of drugs use before(from July 2013 to June 2014)and after(from August 2014 to July 2015) the RUDM were collected. Drug cost ratio the qualified rate of prescripations,the use of antimicrobial agents and auxiliary drugs and drugs for insured patients were evaluated retrospectively. Then the changes of the use of drugs were analyzed before and after the implementation of the RUDM. Results After the RUDM, drug cost ratio was regulated to about 37%; the qualified rate of the prescriptions of outpatients, emergencies and inpatients were increased to 94.21%, 98.79% and 98.89% respectively; the antimicrobial utilization rate of outpatients, emergencies and inpatients were decreased to 6.96%, 38.40% and 49.65% respectively; the prophylactic use rate of antibiotics for typeⅠincision surgeries was reduced to 28.61%; antibiotics used density was decreased by 26.74%;delivered rate of microbial test sample of antibiotics for the overall, restrictive and special use were improved to40.69%, 53.86% and 72.79% respectively; the constituent ratio of auxiliary drugs consumption was controlled by17.25%; the rate of the refusal of payment to insured patients was near to zero. Conclusion After the RUDM, the use of drug was more standardized and the next administration thought was renewed for our hospital.
