摘要
目的观察重组牛碱性成纤维细胞生长因子眼用凝胶联合重组人表皮生长因子滴眼液治疗角膜深层铁屑异物的疗效。方法选择角膜深层铁屑异物患者180例(180眼),随机分成试验组与对照1组和对照2组,各60例。180例患者均采用环形分离整体剔除术剔除角膜异物,术后试验组滴左氧氟沙星滴眼液、重组人表皮生长因子滴眼液、重组牛碱性成纤维细胞生长因子眼用凝胶,对照1组滴左氧氟沙星滴眼液、重组人表皮生长因子滴眼液,对照2组术后滴左氧氟沙星滴眼液、重组牛碱性成纤维细胞生长因子眼用凝胶,术后第3,5,7,9,12天复诊,观察患者症状、角膜创面修复、角膜荧光素染色等。结果用药第5天总有效率试验组为83.33%,对照1组为66.67%(χ2=32.98,P=0.011),对照2组为68.33%(χ2=31.09,P=0.013)。用药第9天总有效率试验组为96.67%,对照1组为85.00%(χ2=33.26,P=0.014),对照2组为81.67%(χ2=35.47,P=0.011)。平均角膜创面愈合时间试验组为(5.35±0.23)d,对照1组为(7.12±0.25)d(t=2.532,P=0.018),对照2组为(7.36±0.31)d(t=2.653,P=0.016)。结论重组人表皮生长因子滴眼液与重组牛碱性成纤维细胞生长因子眼用凝胶联合应用治疗角膜深层铁屑异物疗效显著,优于单用其中1种,能加速角膜创面修复,值得临床推广。
Objective To observe the clinical effect of epidemic growth factor combined with basic fibroblast growth factor in treating deep corneal iron foreign bodies. Methods 180 eyes of 180 patients with deep corneal iron foreign body were randomly divided into experimental group and control group 1 & 2,60 eyes in each group. 180 cases underwent annular separation whole removal,then experimental group were given levofloxacin eye drops and epidemic growth factor eye drops and basic fibroblast growth factor gel for eye. Control group 1 was given levofloxacin eye drops and epidemic growth factor eye drops without basic fibroblast growth factor gel for eye. Control group 2 was given levofloxacin eye drops and basic fibroblast growth factor gel for eye without epidemic growth factor eye drops. Patients visited on the 3rd,5th,7th,9th,12 th day after removal of corneal foreign bodies and the ocular symptoms,wound healing of the cornea,corneal fluorescein staining were observed. Results On the 5th day after removal of deep corneal foreign body,the total effective rate in experiment group was 83. 33%,and 66. 67% in control group 1( χ2= 32. 98, P = 0. 011),and 68. 33% in control group2,compare with the experimental group( χ2= 31. 09, P = 0. 013). On the 9th day after removal of corneal foreign body,the total effective rate in experiment group was 96. 67%,and 85. 00% in control group 1( χ2= 33. 26, P = 0. 014),and 81. 67% in control group 2,compare with the experimental group( χ2= 35. 47, P = 0. 011). Corneal healing time on average of experimental group was( 5. 35 ± 0. 23) d,and( 7. 12 ± 0. 25) d in control group 1( t = 2. 532, P = 0. 018),and( 7. 36 ± 0. 31) d in control group 2,compare with the experimental group( t = 2. 653, P = 0. 016). Conclusion The combination of epidemic growth factor eye drops and basic fibroblast growth factor gel for eye is more effect than they are used alone in treatment of deep corneal iron foreign bodies. They can significantly speed up the repair of corneal wound healing,the effect is remarkable.
