高效抗反转录病毒治疗联合中药治疗艾滋病的临床疗效观察

Observation of the clinical treatment effect on AIDS resulting from the combination of highly active antiretroviral therapy and traditional Chinese medicine

目的观察高效抗反转录病毒治疗(highly active antiretroviral therapy,HAART)联合中药治疗艾滋病疗效。方法回顾分析南宁市第四人民医院2009年6月-2013年6月收治的200例符合入组标准的艾滋病患者作为观察对象,其中观察组100例,对照组100例。对照组单纯采用HAART治疗(齐多夫定300 mg+拉米夫定150 mg+奈韦拉平200 mg,2次/d),观察组在对照组的基础上给予扶正解毒方中药治疗,早晚服用。两组治疗30 d为1个疗程,均治疗6个疗程。观察患者症状体征改善情况,治疗前后采用流式细胞计数测定CD4+,采用RT-PCR检测病毒载量。将数据录入SPSS 17.0软件进行统计分析,采用t检验或χ2检验,P<0.05为差异有统计学意义。结果观察组乏力、肌关节疼痛、皮肤瘙痒、气短等治疗效果明显优于对照组,差异有统计学意义(均P<0.05)。观察组和对照组治疗后CD4+绝对计数较治疗前升高(均P<0.05),差异有统计学意义;观察组治疗后CD4+绝对计数高于对照组(P<0.05),差异有统计学意义。观察组治疗后病毒载量较治疗前降低,差异有统计学意义(P<0.05);对照组治疗前后病毒载量差异无统计学意义(P>0.05)。结论 HAART联合扶正解毒中药能够提高CD4+数量,促进免疫重建,降低病毒载量,可在临床积极推广。

Objective To research the effect on AIDS resulting from the combination of highly active antiretroviral therapy(HAART) and traditional Chinese medicine.Methods This study was a retrospective analysis of 200 AIDS patients who treated at Nanning Fourth People's Hospital between June 2009 and June 2013 and also met the study inclusion criteria 100 cases were assigned to observation group and100 cases were assigned to control group.The patients in control group were given only HAART [Azidothymidine(AZT) 300 mg + Lamivudine(3TC) 150 mg + Nevirapine(NVP) 200 mg,twice per day],the patients in observation group were given Fuzhengjiedu traditional Chinese medicine on the basis of control group,twice per day.30 d was a course of treatment,and both groups had been treated for 6month.Symptoms and signs were observed through study.CD4+was measured using flow cytometry method before and after treatment,and viral load were measured using RT-PCR method.SSPS 17.0was used to analyze data,t test and χ2test was used to compare groups,and p-value<0.05 was counted as statistically significant.Result The effects of reducing fatigue,muscle and joint pain,itching,shortness of breath in observation group were significantly better than in control group(P<0.05).CD4 +counts in both groups after treatment were significantly higher than before treatment(P<0.05).CD4+counts after treatment in observation group were significantly higher than control group(P<0.05).The viral load after treatment in observation group wassignificantly lower than before treatment(P<0.05).The viral load of the control group had no significant difference before and after the treatment(P>0.05).Conclusions HAART combined with Fuzhengjiedu medication can increase the number of CD4+,promote immune system reconstitution,and reduce viral load in the treatment of AIDS.This strategy should be promoted in clinical.
