血钠、血B型利钠钛、尿酸在女性心力衰竭患者血清中的表达及相关性分析

Correlation analysis for serum sodium,serum brain-type natriuretic peptide(BNP),and serum uric acid among female patients with heart failure

摘要 目的探讨血钠(Na)、B型利钠尿肽(B3NP)、尿酸(UA)与女性心力衰竭的关系。方法选取2010年1月至2012年12期间120例女性心力衰竭患者为研究对象(观察组)。将120例心力衰竭患者按照病情程度的不同分为心力衰竭急性期(68例)和心力衰竭稳定期(52例);将120例患者按照NYHA分级标准^([1])分为心力衰竭心功能Ⅱ级48例,Ⅲ级40例,Ⅳ级32例。另选择同期在嘉兴市秀洲区新塍医院门诊体检的健康女性人群84例为对照组。统计受试者血Na、BNP、UA水平,并对心力衰竭患者血Na、BNP、UA水平与心力衰竭级别进行相关性分析。采用SPSS 19.0软件,计量资料用t检验或单因素方差分析,相关性分析采用Pearson检验。P<0.05为差异有统计学意义。结果观察组血BNP、UA水平高于对照组,血Na水平明显低于对照组,差异有统计学差异(均P<0.01)。心力衰竭急性期患者血BNP、UA水平高于心力衰竭稳定期,血Na水平明显低于心力衰竭稳定期,差异有统计学差异(均P<0.01)。不同心功能分级心力衰竭之间血清BNP、UA水平存在明显差异。随心力衰竭心功能级别的增加,血清BNP、UA水平呈上升趋势,且3组间两两比较均有统计学差异(P<0.01);随心力衰竭心功能级别增加,血Na水平呈下降趋势,且3组间两两比较均有统计学差异(P<0.01);血Na与女性心力衰竭心功能分级呈明显负相关(r=-0.431,P=0.007);BNP、UA与女性心力衰竭心功能分级呈明显正相关(r=0.501,0.478,P=0.003,0.012)。结论血BNP、UA在女性心力衰竭中呈现高表达,而血Na在女性心力衰竭中呈现下降趋势,血Na、BNP、UA与女性心力衰竭病情及严重程度密切相关。 Objective To study the relationships between serum sodium,serum BNP,and serum uric acid(UA) among female patients with heart failure.Methods 120 cases of female heart failure patients(observation group) were recruited from January 2010 to December 2012.According to the severity of the diseases,these 120 patients were divided into acute heart failure group(68 cases) and stable heart failure group(52 cases).120 patients were graded according to the New York Heart Association standards.48 cases had grade Ⅱ heart failure,40 cases had grade Ⅲ,and 32 cases had grade Ⅳ.84 healthy women were recruited as control group during the same time period from the outpatients of our hospital during the same period in our hospital.Serum sodium,serum BNP,and serum UA of participants were measured and the correlations between them were analyzed.SPSS 19.0 software was used to analyze the data.Continuous variables were compared between groups using t— test and single factor analysis of variance;categorical variables were compared using Pearson correlation test.Results Serum BNP and UA levels among observation group were significantly higher compared to the control group,and serum sodium levels were significantly lower(P<0.01).Serum BNP and UA levels among patients with acute heart failure were significantly higher than among patients with stable heart failure,and serum sodium levels were significantly lower(P<0.01).There were clear differences on the levels of serum BNP and UA between different grades of heart failure.The worse heart failure was,the higher serum BNP and UA levels were.The differences were statistical difference(P<0.01).The worse heart failure was,the lower serum sodium levels were.The differences were also statistical difference(P<0.01).Serum sodium levels and female heart failure cardiac functional grades showed a negative correlation(r=-0.431,P=0.007);BNP and UA showed positive correlation with female heart failure cardiac function grades(r=0.501,0.478,P=0.003,0.012).Conclusions BNP and UA are highly expressed among elderly female patients with heart failure,but serum sodium showed low expression.Serum sodium,BNP and UA are closely related to the female congestive heart failure and the severity of illness.
