左卡尼汀注射液治疗老年冠心病心力衰竭的临床疗效

Clinical effect of levocarnitine injection on treatment of elderly patients with coronary heart disease and heart failure

目的分析研讨左卡尼汀注射液治疗老年冠心病心力衰竭(简称"心衰")的临床疗效。方法选取68例2014年7月—2015年12月衡阳市中心医院收治的老年冠心病心衰患者,随机分为对照组和观察组,各34例,对照组接受常规治疗(硝酸脂类药物、利尿剂、血管紧张素转换酶抑制剂及β受体阻滞剂等),观察组在对照组治疗基础上加用左卡尼汀注射液。将两组患者的临床疗效、左室舒张末期容积(LVEDD)、左室收缩末期容积(LVESD)、左室射血分数(EF)、心输血量(CO)等心功能指标及空腹血糖(FPG)、餐后血糖(PPG)、糖化血红蛋白(Hb A1c)水平进行对比分析。结果经治疗后,观察组(94.12%)总有效率明显高于对照组(79.14%),差异有统计学意义(P<0.05);观察组患者的LVEDD、LVESD、EF、CO等心功能指标改善情况明显优于对照组,差异有统计学意义(P<0.05);观察组FPG、PPG、Hb A1c水平均明显优于对照组,差异有统计学意义(P<0.05)。结论左卡尼汀注射液治疗老年冠心病心力衰竭,其疗效确切,可明显改善患者的心功能,值得临床应用并推广。

Objective To investigate the clinical effect of levocarnitine injection on treatment of elderly patients with coronary heart disease and heart failure. Methods A total of 68 elderly patients with coronary heart disease and heart failure in Central Hospital of Hengyang from July, 2014 to December,2015 were selected and randomly divided into the control group and the study group. Patients in the control group were given the routine treatment including nitrates drugs, diuretics and β-blocker, etc.,while patients in the study group were treated with the levocarnitine injection on the base of the routine treatment. Then, the left ventricular end diastolic volume(LVEDD), left ventricular end systolic volume(LVESD), left ventricular ejection fraction(EF), cardiac output(CO), fasting plasma glucose(FPG),postprandial plasma glucose(PPG), Hb A1 clevels were compared between the two groups. Results After treatment, the total effective rate in the study group was 94.12%, which was significantly higher than that in the control group(79.14%, P<0.05). LVEDD, LVESD, EF, CO in the study group were significantly better than those in the control group(all P<0.05). The levels of FPG, PPG, Hb A1 cin the study group were significantly better than those in the control group(all P<0.05). Conclusion Levocarnitine injection is effective for the treatment of elderly patients with coronary heart disease and heart failure, and it can significantly improve the patient's heart function, thus it is worthy to be applied and promoted in the clinic.
