幽门螺旋杆菌和慢性胃炎胃黏膜病理变化的相关性研究

Correlation between helicobacter pylori and pathological changes of gastric mucosa in patients with chronic gastritis

目的探讨幽门螺旋杆菌(Hp)感染和慢性胃炎(CG)患者胃黏膜病理变化的相互关系,为临床治疗提供借鉴依据。方法选取2015年2月—2017年2月许昌市中心医院收治的68例CG患者作为观察组,44例同期健康体检者为对照组,对两组进行Hp阳性检查,并分析CG患者炎症程度、萎缩程度和肠化分级不同病理特征的Hp感染情况,分析比较Hp阳性率。结果观察组Hp阳性率为84.82%,显著高于对照组的38.39%,差异有统计学意义(P<0.05);随CG炎症程度、萎缩程度、肠化分级等不同病理特征的加剧,Hp阳性率均逐渐升高,差异有统计学意义(P<0.05)。结论 Hp和CG胃黏膜患者病理变化有一定相关性,可反映出患者炎症感染程度、萎缩程度和肠化分级程度。

Objective To investigate correlation between pathological changes of gastric mucosa in patients with helicobacter pylori(Hp) infection and chronic gastritis(CG) in order to provide reference for clinical treatment. Methods Totally 68 patients with CG treated in Xuchang Central Hospital from February 2015 to February 2017 were selected as the observation group, and 112 healthy subjects were enrolled as control group at the same time. Hp positive examinations were performed in two groups and the degrees of inflammation and atrophy and Hp infection in different pathological grades of intestinal metaplasia of CG patients were analyzed. The positive rate of Hp was compared. Results The positive rate of Hp in the observation group was 84.82%, which was significantly higher than 38.39%in the control group, and the difference was statistically significant(P<0.05). With the increase of different pathological features, such as CG degree of inflammation, extent of atrophy and grading of intestinal metaplasia, the positive rate of Hp was increased gradually and the difference was statistically significant(P<0.05). Conclusion Hp has a certain correlation with pathological changes of gastric mucosa in patients with CG, which can reflect the degrees of inflammation, atrophy and intestinal metaplasia.
