心房颤动对QT间期的影响

EFFECT OF ATRIAL FIBRILLATION ON QT INTERVAL

对30例房颤病人的50个心动每一QT间期与前一RR间期、平均QT与平均RR进行了相关性研究。结果表明:平均QT与平均RR呈线性相关,QT=169+0.253RR(P<0.001,r=0.892)。各心动的QT与前一RR也呈线性正相关,其平均回归斜率为0.081。每例各心动QT和前一RR的回归斜率与平均RR则呈线性负相关(r=-0.620,p<0.05)。30例的QT较同一平均心室率时窦性心律延长,除3例外其余仍在Lepeshkin报道正常范围,而平均QT与RR回归线高于Ahave报道的冠心病患者窦性心律的回归线。作者认为,QT间期不仅受病因、心室率的影响,而且受心律影响,故不宜单纯用Bazett公式纠正房颤病人(尤其在快速房颤时)的QT间期。

The correlation research between each QT interval and the proceeding RR interval, the mean QT and mean RR in 50 heart beats was performed on 30 patients with atrial fibrillation. The results indicated that the mean QT interval was linearly related to the mean RR interval ( QT=169 + 0.253, P<0.001, r =0.892). QT of each heart beat was linearly related to the proceeding RR, too. Its average regression slope was 0.081. The regression slope of QT interval in each heart beat and the proceeding RR was negative, linearly corelated to the mean RR(r = -0.620, P <0.05 ) . The mean QT in all subjects was longer than that in the subjects with the same ventricular rates in sinus rhythm. Except for 3 cases all were in the normal range reported by Lepeshdin. The regression line of the mean QT and the mean RR was above the regression line in sinus rhythm with the coronary heart disease reported by Ahave. The authors considered that the QT interval was affected not only by the etiology and ventricular rate, but also the heart rhythm. So a simple correction of QT interval for heart rate by Bazett's formula is inadequate, especially in the fast ventricular rate in atrial fibrillation.
