摘要
目的:通过临床观察中医活血通络干预治疗小儿哮喘与甲皱微循环的关系。方法:按全国儿科哮喘防治协作组及《中药新药治疗哮证的临床研究指导原则》的诊断标准选择哮喘发作期(热型)患儿60例,随机分为治疗组和对照组,观察两组治疗前后甲皱微循环的变化。结果:治疗组的临床疗效明显优于对照组(P<0.05);治疗组的甲皱微循环加权积分明显低于对照组(P<0.01)。结论:小儿哮喘发作期(热型)患儿存在瘀血阻络的病理基础,应用活血化瘀中药可提高临床疗效;甲皱微循环检查可作为小儿哮喘发作期(热型)中医辨证和疗效判断的客观指标;活血通络中药“喘通康”达到控制炎症,降低气道反应性,从而缓解支气管哮喘的目的。
Objective: To observe clinically the relationship between the intervention of TCM promoting blood circulation and dredging collaterals in the treatment of infantile asthma and nail fold microcirculation. Methods: 60 cases(heat type) of asthma at attack stage were selected according the Diagnosis Standards of the National Collaboration Group for the Prevention and Treatment of Infantile Asthma as well as Clinical Research Guidelines of Treatment of Asthma with New Traditional Chinese Drugs. The cases were randomly divided into the treatment group and the control group to observe the changes of the nail fold microcirculation in both groups before and after the treatment. Results: The clinical therapeutic effect of the treatment group was markedly better than that of the control group(P<0. 05) , while the weighted integral of the nail fold micro circulation in the treatment group was markedly lower than that of the control group(P<0.01). Conclusion: Infantile asthma patients at attack stage(heat type) exist the pathological basis of blood stasis obstructing collaterals. The application of traditional Chinese drugs for promoting blood circulation and dredging collaterals may improve clinical therapeutic effects; Examination of nail fold microcirculation may be used as the objective indexes of TCM syndrome differentiation and evaluation for infantile asthma at attack stage (heat type) ; the TCM drug for promoting blood circulation and dredging collaterals'Chuan Tong Kang' may achieve the purposes of controlling inflammation and reduce reactivity of air passage so as to relive bronchial asthma.
