血管内超声对冠状动脉临界病变处理的应用价值 被引量:6

摘要 目的 探讨血管内超声 (IVUS)在判断冠状动脉造影临界病变介入治疗指征中的价值。方法 对 6 8例稳定型心绞痛 (SA)患者 (2 6例 )和不稳定型心绞痛 (UA)患者 4 2例做冠状动脉造影 (CAG) ,显示单支临界病变的患者行IVUS检查 ,以面积狭窄 6 0 %或偏心脂质斑块作为介入治疗指征。结果  6 8处病变进行介入治疗 5 0处(73.5 3% ) ,对于血管直径的判断 ,IVUS优于CAG ;对于有价值病变的检出 ,UA组优于SA组。结论 对于CAG显示的临界病变 ,尤其是表现为UA者 。 Objective To evaluate the role of intravascular ultrasound(IVUS) in determining strategy for coronary artery stenosis of intermediate severity.Methods IVUS was performed in 68 patients who had lesions of intermediate severity(40%-70% diameter stenosis) in coronary angiography(CAG),percent area stenosis (A%)>60% or eccentric-soft plaque was used as an indication of coronary stenting.Results 50 segments( 73.53%) underwent interventional treatment.In judging vessel diameter,IVUS is superior to CAG;it is also for selecting indications of stenting,especially for those patients with UA.Conclusion IVUS is valuable in decision making for intermediate coronary stenosis,especially UA patients.
