基质金属蛋白酶(MMP-9)及其抑制剂(TIMP-1)在食管鳞癌中表达的临床意义

Studies on the Clinical Significance of Matrix Metalloproteinase-9 and Tissue Inhibitors of Metalloproteinase-1 Expression in Esophageal Sqamous Carcinoma

目的:探讨MMP-9及TIMP-1在食管鳞癌中表达的临床意义。方法:采用SP法检测86例食管鳞癌组织及42例正常食管粘膜组织中MMP-9、TIMP-1的表达情况。结果:1)42例正常食管粘膜组织表达阴性。2)MMP-9在食管鳞癌组织中的阳性表达率为65.12%。其中高、中分化和低分化阳性表达率分别为51.06%和82.05%(P<0.01);Ⅰ、Ⅱ期患者和Ⅲ、Ⅳ期患者分别为42.86%和80.39%(P<0.01);淋巴结转移组和无转移组分别为75.81%和37.50%(P<0.01)。3)TIMP-1在食管鳞癌组织中表达的阳性率为39.02%。其中高、中分化和低分化者阳性表达率分别为44.67%和28.21%(P<0.05);Ⅰ、Ⅱ期患者和Ⅲ、Ⅳ期患者分别为54.29%和25.49%(P<0.01);淋巴结转移组和无淋巴结转移组分别为22.58%和75.00%(P<0.01)。4)Kaplan-Meier分析,MMP-9高表达和TIMP-1低表达患者的生存时间明显低于MMP-9低表达和TIMP-1高表达患者(P<0.05)。结论:MMP-9和TIMP-1的表达与食管鳞癌关系密切,对判断其预后可能具有重要意义。

Objective:Studies on the clinical significance of matrix metalloproteinase-9and tissue inhibitors of metalloproteinase-1expression in esophageal sqamous carcinoma ESC.Meth-ods:The expression of MMP-9and TIMP-1of86cases of ESC and that of42cases of nor-mal esophageal mucosa were detected by the method of streptaridin-peroxidase,Results:①There were no expression of MMP-9and TIMP-1in normal esophageal mucosa of42cases.②The ex-pressive rate of MMP-9of ESC was65.12%.The expression of Well-mediate differentiation and poor were51.06%.and82.05%(P<0.01);Ⅰ-Ⅱgrade andⅢ-Ⅳgrade were42.86%and80.39%(P<0.01);The expression of lymph node metastasis and no lymph node metastasis were75.81%and37.50%(P<0.01).③The expressive rate of TIMP-1of ESC was39.02%.The expression of well-mediate differentiaton and poor were44.67%and28.21%(P<0.05);Ⅰ-Ⅱgrade andⅢ-Ⅳgrade were54.29%and25.41%(P<0.01);The expression of lymph node metastasis and no lymph node metastasis were22.58%and75.00%(P<0.01).④Kaplan-Meier show the survival time of high expression of MMP-9and low expression of TIMP-1was lower than that of low expression of MMP-9and high espression of TIMP-1(P<0.05).Conclusion:the expressive rate of MMP-9and TIMP-1was closely related to the esophageal squamous carcinoma,and would be a valuable prognostic marker.
