缺碘孕妇孕期补碘的安全性与必要性的初步探讨

A first-step investigation to study the safety and the necessity of iodine deficiency pregnant women intake tablets with iodine during pregnancy

目的 了解武汉地区孕妇的碘营养状况及探讨缺碘孕妇孕期补碘的安全性与必要性。方法 尿碘含量测定采用尿碘快速定量检测法。结果 ①非孕妇尿碘值中位数为 2 5 2 .36 μg/L ,缺碘率为 0 % ,妊娠早、中、晚期尿碘的中位数分别为 173.77μg/L、186 .81μg/L、184 .79μg/L ,妊娠早期尿碘的中位数较低 (P <0 .0 5 ) ;缺碘率分别为 6 5 .4 3%、5 4 .32 %、5 0 .0 0 % ;妊娠早期缺碘率较高 (P <0 .0 5 )。②妊娠中期补碘组及未补碘组尿碘中位数分别为 2 10 .87g/L、14 2 .97μg/L ,缺碘率分别为 2 8.13%、71.4 3% ,差别有显著性 (P <0 .0 0 1) ;妊娠晚期补碘组及未补碘组尿碘中位数分别为 2 4 8.70 μg/L、175 .4 5μg/L ,缺碘率分别为 2 3.5 3%、6 6 .6 7% ,差别有显著性 (P <0 .0 0 1)。结论 武汉地区单纯食用合格碘盐并不能满足孕妇的需要 ,孕妇孕期补碘 (15 0 μg/d)是安全的也是必要的。

Objective: To acquire the data of iodine value from pregnant women in Wuhan city and investigate the necessity and the safety of intake iodine daily(150μg/d) during pregnancy. Methods: urine iodine is tested by fast urine iodine quantity method. Results: The median of urine iodine value is 252.36 μg/L, and iodine deficiency incidence is 0% from non-prgnant women. The median of urine iodine value is 173.77μg/l, 186.81μg/l and 164.79μg/l, and iodine deficiency incidence is 65.43%, 54.32% and 50.00% from first, second and third trimester respectively. In the first trimester pregnancy urine iodine value mean is significantly lowest and iodine deficiency incidence is significantly highest.(Ρ<0.05). The median of urine iodine value is 210.87μg/L? 142.97μg/L and iodine deficiency is 28.13%?71.43% from second trimester of taking iodine group and not taking iodine group respectively; The third trimester is 248.70μg/L?175.45μg/L; 23.53%?66.67% respectively,the difference is significantly.(P<0.001) Conclusions: Iodine intakes daily from the qualified iodized salt cannot satisfy the needs of the pregnant women living in Wuhan. A method of regular intake tablets with iodine (150μg/d) during pregnancy is necessary and safe.
