电视腹腔镜下子宫次全切除术60例临床探讨

Evaluate of 60 Patients Laparoscopic Supracervical Hysteretomy

目的 探讨腹腔镜子宫次全切除术代替经腹子宫次全切除术的可能性、腹腔镜子宫次全切除术的手术要点及临床应用价值。方法 我院 2 0 0 2年 3月—2 0 0 2年 1 2月腹腔镜子宫次全切除术 60例。对 60例因各种良性妇科疾病而需行子宫次全切除术的患者 ,采用腹腔镜子宫次全切除术式。患者平均年龄 44岁 ( 3 9～ 68岁 ) ,术前子宫正常大小者 1 2例 ,子宫增大、小于孕 1 2周 3 8例 ,大于孕 1 2周 1 0例 ,其中有下腹手术史者 1 2例。结果 60例行腹腔镜子宫次全切除术中 ,2例中转开腹手术 ,中转率 3 .3 %。平均手术时间 80min ( 4 0～2 40min) ,出血量约 5 0～ 1 5 0ml。平均住院日为 6d。结论 腹腔镜子宫次全切除术基本可代替经腹子宫次全切除术 ,可避免开腹、减少手术创伤。

Objective To evaluate the possibility that the laparoscopic supracervical hysterectomy replaces the laparotomy supracervical hysterectomy.The technical procedure and the clinical value of laparoscopic supracervical hysterectomy.Methods March 2002 to December 2002,60 laparoscopic supracervical hysterectomy in our hospital.Sixty patients suffered from various kinds of benign gynecological diseases underwent laparoscopic supracervical hysterectomy . The mean age was 44 years (range 39～68), preoperative examination findings were 12 cases of normal uterine size, 48 of uterine enlargement (38 less than 12 week gestational size and 10 larger). Among these patients, 12 cases had a history of lower abdominal laparotomy.Results 2 out of the 60 cases were switched to laparotomy (3.33%). Mean operation time of laparoscopic supracervical hysterectomy was 80 min (range 40～240 min). The amount of blood loss was approximately 50～150 ml during operation, and the average hospitalization was 6 days.Conclusion The laparoscopic supracervical hysterectomy replaces the laparotomy supracervical hysterectomy basically,avoids laparotomy and lessns injuries.
