胰岛素瘤的定位诊断及外科治疗方法探讨

Localization and surgical treatment of insulinoma

目的 探讨胰岛素瘤的定位诊断及外科治疗方法。方法 对确诊的 18例胰岛素瘤患者的临床资料进行回顾性分析。结果 术前 B超、CT和选择性动脉造影检查的阳性率分别为 4 6 % (7/ 15 )、5 4 % (7/ 13)和 75 %(6 / 8) ,术中 B超检查的阳性率为 10 0 % (5 / 5 )。 18例中行肿瘤剜除术 11例 ,胰体尾切除术 7例。结论 胰岛素瘤的术前定位诊断比较困难 ,术中探查和术中 B超检查准确性很高 ;

Objective To study the localizating methods and treatment of insulinoma.Methods The clinical data of 18 cases were reviewed.Results The positive rate in detection of insulinoma by ultrasonography,CT scan,selected arteriography and intraoperative ultrasonography was 46%(7/15),54%(7/13),75%(6/8) and 100%(5/5)respectively.11 cases and 7 cases were performed enucleation and distal pancreatectomy respectively.Conclusions The preoperative diagnosis of insulinoma is difficult.The intraoperative palpation and ultrasonography can get high accuracy.The resection of the tumors is the only reliable type of treatment to insulinoma.
