突眼患者血中的细胞因子mRNA表达

摘要 目的 探讨Graves突眼的细胞因子表达。方法 门诊就诊的突眼患者47例,其中合并甲亢者30例,无甲亢者17例,正常对照30例。取外周血,分离单个核细胞,用逆转录聚合酶链反应(RT-PCR)检测6种细胞因子(IFN-γ、TNF-α、IL-2、IL-4、IL-6、IL-10)mRNA的转录。结果 突眼并甲亢组Th_1类和Th_2类细胞因子的表达率的差别无显著意义;单纯突眼组Th_1类细胞因子表达率(51.0%)明显高于Th_2类细胞因子的表达率(7.8%)(P<0.01)。突眼并甲亢组和单纯突眼组Th_1类细胞因子的表达率分别为53.3%和51.0%,差别无显著性意义;突眼并甲亢组Th_2类细胞因子的表达率明显高于单纯突眼组(P<0.05)。IFN-γ的表达率在单纯突眼组和突眼并甲亢组的差别无显著性意义,IL-4的表达率,单纯突眼组低于突眼并甲亢组,差别有显著性意义(P<0.01)。结论 甲亢和突眼发病的免疫学模式不同,不同突眼患者细胞因子的表达各异。 Objective To study the cytokine mRNA expression of mononuclear cells in peripheral blood of patients with Graves' ophthalmopathy. Methods The blood samples of 47 cases with Graves' ophthalmopathy, including 30 cases with and 17 cases without hyperthyroidism and 30 normal people, was obtained ,and the monocyte was isolated. The mRNA transcriptions of 6 kinds of cytokine (IFN--y ,TNF-a,IL-2 ,IL-4 ,IL-6,IL-10) were detected by respective reserve transcription-polymerase chain reaction( RT-PCR ). Results There was not remarked difference in expression rate of Th1 and Th2-like cytokines in ophthalmopathy associated with hyperthyroidism group ( P > 0. 05 ) , and Th1 -like cytokines between ophthalmopathy with and without hyperthyroidism (P > 0. 05) , but the expression rate of Th1 -like cytokine was significantly higher than that of Th2-Iike cytokine in ophthalmopathy without hyperthyroidism(P< 0. 01). Compared with ophthalmopathy without hyperthyroidism group, the expression rate of Th1-like cytokines and IFN-y was not different in ophthalmopathy with hyperthyroidism group( P >0. 05) , but the expression rate of Th2-like cytokines and IL-4 was increased (P <0. 01). Conclusion There are some differences of immunological pattern in pathogenesis between hyperthyroidism and ophthalmopathy as well as various expression of cytokine between ophthalmopathy with and without hyperthyroidism.
