摘要
目的 :研究高氧液 (HO)对家兔油酸型急性肺损伤(ALI)的治疗作用 .方法 :以林格氏液为基液制备HO .将 18只家兔随机分为油酸组 (A组 ) ,高氧液组 (B组 )和正常对照组 (C组 ) ,每组 6只 .A和B组均经耳缘静脉注入油酸 0 .0 6mL·kg-1建立ALI模型 ,C组动物注入等量生理盐水 .B组于注射油酸 30min时经右侧颈外静脉给予HO 2 0mL·kg-1,A、C组给予等量林格氏液 .各组分别在ALI模型制备前 (0min)、注射油酸后 30 ,6 0 ,12 0min时间点经右颈外动脉进行血气分析 ,2h后处死动物取肺组织 ,测定肺组织髓过氧化物酶 (MPO)含量、肺水含量 ,肺体质量比值及观察病理变化 .结果 :与C组相比A组PaO2 显著降低 (P <0 .0 1) ,髓过氧化物酶、肺水含量及肺体质量比值显著增高 (P <0 .0 1) ,镜下见肺灶性出血 ,间质、肺泡水肿 ,大量炎细胞浸润 .B组经HO治疗后迅速提升PaO2 (P <0 .0 5 ) ,降低髓过氧化物酶含量 ,减轻肺水肿 (P <0 .0 1) ,大部分肺组织无明显病理学改变 .结论 :HO对油酸型ALI家兔有治疗作用 .
AIM: To investigate the effect of hyperoxia solution (HO) on acute lung injury ALI in rabbits induced by oleic acid. METHODS: HO was prepared from Ringers solution. Eighteen rabbits were randomly divided into three groups: Oleic acid group ( group A), HO treatment group( group B) and control group( group C) with 6 in each group. ALI model was established by iv oleic acid (0.06 mL·kg -1 ) in group A and group B, while in group C, the same volume of normal saline was given. At 30 min after oleic acid administration, HO (20 mL·kg -1 ) was applied in group B, which was replaced by Ringers solution in group A and group C. Blood samples were taken for blood gas analysis before and at 30, 60, 120 min after oleic acid or normal saline administration. Two hours later, the animals were suicided, and myeloperoxidase (MPO), content of lung water, lung/body ratio were measured, and pathologic changes of lung tissue were observed microscopically. RESULTS: Compared with group C, PaO 2 significantly decreased ( P <0.01), while MPO, content of lung water, and lung/body ratio markedly increased ( P <0.01), and local hemorrhage, interstitial edema, alveoli exudation, and massive inflammatory cells infiltration were also observed in group A. In group B, PaO 2 significantly increased ( P < 0.05), MPO and content of lung water decreased ( P < 0.01), and only mild pathological changes were observed.
