扣带回肿瘤的临床特征及其微创手术治疗

The clinical features and microsurgery of the lesions in the cingulate gyrus

目的总结扣带回肿瘤的临床特点和微创手术治疗效果。方法回顾性分析2001年1月～2003年4月经手术治疗的27例扣带回肿瘤。结果病人均以癫为首发症状;手术全切22例,近全切除5例;病理为星形细胞瘤17例,少枝胶质瘤5例,混合性胶质瘤3例,海绵状血管瘤和胶质增生各1例。结论扣带回肿瘤好发于中青年人,常以癫为首发症状,肿瘤局限于扣带回内生长,一般不侵犯邻近的区域,病理上以低分级胶质瘤为主,沿纵裂入路切除肿瘤可以减少手术创伤,安全切除病变。

Objective To summarize the clinical features and our experience of surgical skills on lesions of the cingulate gyrus. Methods The clinical manifestation and pathological characters of 27 cases of lesions of the cingulate gyrus were studied retrospectively. Results Seizure occurred as an initial symptom in all of the 27 cases. Pathology examination showed astrocytoma in 17 cases, oligodendroglioma in 5 cases, mixed-glioma in 3 cases, cavernous hemangioma in 1 case and gliosis in 1 case. Conclusion Seizure is usually the initial manifestation of lesions of the cingulate gyrus. Low-grade glioma and other benign lesions are the major pathological findings. Trans-interhemispheric-fissure approach is the most suitable way in treating lesions of the cingulate gyrus.
