偶极子定位系统在癫癎外科中的应用

摘要 目的 探讨偶极子定位 (Dipolelocalizationmodeling ,DLM)系统对致疒间 灶定位的价值。方法 对 82例拟行癫疒间病灶切除术的患者进行DLM和MRI检查 ,部分病人行SPECT PET检查 ;79例手术病人的皮层或深部脑电图定位与术前DLM进行对比分析。结果 DLM检查与术中定位结果基本一致 ,且特异性好。结论 DLM是一种新型无创性疒间 灶定位方法 ,因其特异性定位优于影像学方法 。 Objective To investigate the value of localization of epileptogenic foci by dipole localization modeling (DLM). Methods Eighty two patients with epilepsy received DLM and MRI examinations, and part of the patients received examination by SPECT/PET. Results of intraoperative localization by cortical electrogram or depth electroencephalogram in 79 patients were compared with those by preoperative DLM. Results The results of localization by DLM were identical to those by intraoperative localization, but were of more specificity. Conclusion DLM is a new method for the non invasive localization of epileptic foci due to its better specific localization than imaging, suggesting that DLM localization may play an important role in the surgical treatment of intractable epilepsy.
