动脉搏动指数对评价糖尿病脑血管病变的意义 被引量:6

摘要 目的 采用经颅多普勒 (TCD)评价糖尿病患者的脑血流动力学改变。 方法 对 1 2 0例无脑卒中、血压正常的 2型糖尿病 (T2 DM)患者 ,1 0 0名年龄和性别相匹配的健康志愿者 ,检测了大脑中动脉 (MCA)、颈内动脉颅外段 (ICA)、基底动脉 (BA)的血流速度和 Gosling搏动指数 (PI)。 结果 糖尿病组 ICA血流速度明显高于对照组。有并发症患者 MCA和 ICA的 PI明显高于无并发症患者和对照组 ,BA的 PI也显著高于对照组。PI与糖尿病病程密切相关 (r=0 .4 8)。 结论 糖尿病患者脑血流动力学改变的 TCD所见 ,提示 Objective This study was designed to evaluate cerebral hemodynamic changes related to diabetes mellitus (DM) with transcranial Doppler (TCD). Methods We measured the flow velocities and the Gosling pulsatility index(PI) of the middle cerebral artery (MCA),extracranial internal carotid artery (ICA),and basilar artery (BA) in 120 stroke free,normotensive patients with type 2 DM and 100 age and gender matched healthy volunteers. Results The flow velocity of ICA in patients was significantly higher than that in controls. The PIs of the MCA and ICA were significantly higher in patients with complication than those without complication,as well as in controls.The PI of the BA was also significantly higher than in controls.The PI were closely correlated with the duration of DM ( r =0.48). Conclusion This study defines TCD finding of diabetes related cerebral hemodynamic changes and suggests that the PI reflects pathologic changes of cerebral vessels.
