摘要
目的 探讨术前血清血管内皮生长因子 (VEGF)检测在卵巢上皮性肿瘤诊断及预后判断方面的价值。方法 利用酶联免疫吸附分析 (ELISA)检测卵巢上皮性肿瘤患者 (恶性 4 7例 ,良性 14例 )术前血清VEGF浓度 ,11例正常妇女的血清作为对照。分析检测结果与临床资料的关系。结果 血清VEGF检测结果 ,恶性组血清VEGF均值 2 2 1.81pg/ml(95 %置信区间 (CI) 180 .35~ 2 6 2 .87pg/ml) ,良性组均值 15 8.2 1pg/ml(95 %CI2 7.32~ 2 79.10pg/ml) ,正常对照组均值 12 1.4 8pg/ml(95 %CI 34.13~ 2 2 8.32pg/ml)。三组血清VEGF均数差异无显著性 (P〉0 .0 5 )。 4 7例恶性上皮性卵巢肿瘤中 ,晚期患者 (38例 )的术前血清VEGF浓度比早期患者 (9例 )的高 ,差异有显著性 (P <0 .0 5 )。肿瘤分化程度差 (G3 )的患者术前血清VEGF浓度高于肿瘤分化程度高(G1~ 2 )的 (P <0 .0 5 )。不同病理类型的患者术前血清VEGF水平无统计学差异 (P〉0 .0 5 )。对 2 9例已经进行彻底的肿瘤细胞减灭术的患者随访 ,术前血清VEGF水平高的患者 2年内肿瘤复发或远处转移的可能较大 (P<0 .0 5 )。结论 术前血清VEGF可能对反映患者的预后有一定价值。
Objective: To determine whether serum vascular endothelial growth factor could be used as a diagnostic and prognostic marker in ovarian cancer patients.Methods: Vascular endothelial growth factor in pretreatment serum samples of 61 patients with epithelial ovarian neoplasms (47 malignant and 14 benign)and 11 normal women in control group was measured by enzymelinked immunosorbent assay.Results: The mean preoperative serum VEGF levels in epithelial ovarian carcinoma, in benign disease and in control were 221.81 pg/ml (95% confidence interval (CI) 180.35~262.87 pg/ml), 158.21 pg/ml (95% CI 27.32~279.10 pg/ml) and 121.48 pg/ml (95% CI 34.13~228.32 pg/ml) respectively . But their difference had no statistical significance (P>0.05). Preoperative serum VEGF level in 38 advanced ovarian carcinomas was higher than that in 9 early ovarian carcinomas (P〉0.05) . Serum VEGF level was high in those with poor histological differentiation(G 3) (P〈0.05). Serum VEGF level did not have correlation with histological types (P〉0.05). 29 cases had completely cytoreductive surgery. With their post-operation data, elevated preoperative VEGF serum level was correlated with a poorer outcome in two years (P<0.05) .Conclusions: Vascular endothelial growth factor appears to be an additional factor for predicting prognosis of patients with epithelial ovarian cancer.
