内镜下黏膜色素染色和结扎治疗术对早期胃癌的诊疗价值

Diagnostic and therapeutic values on early carcinoma of stomach by endoscopic dye staining and ligation

目的 探讨内镜下黏膜色素染色和结扎治疗术对早期胃癌的诊疗价值。方法 对胃癌可疑患者局部喷洒美蓝染色剂 ,观察着色情况并取材活检。在黏膜染色的指引下 ,对早期胃癌患者行内镜下结扎治疗术。结果 对胃癌可疑患者 112例行美蓝染色 ,99例 ( 85 .87% )病灶区域染色不均匀或出现重染色区域 ,取病理活检证实早期胃癌 2 7例。术后随访 2年 ,1例于第 3年复发 ,17例未见复发 ;6例经内镜下结扎治疗术后再度行胃癌根治术 ,手术切除标本经连续切片未发现肿瘤细胞 ,随访 3年未见复发。结论 内镜下结合黏膜色素染色和结扎治疗术不仅可以提高早期胃癌的诊断准确性 ,而且操作方法简便易行 ,安全可靠 ,具有推广应用价值。

Objective: To evaluate the diagnostic and therapeutic values on early carcinoma of stomach by endoscopic dye staining and ligation. Methods:Methylene blue was taken as dyes to stain suspected early carcinoma of stomach cases and with the help of mucosa dye staining, endoscopic ligation was performed on early carcinoma of stomach cases. Results:One hundred and twelve suspected early carcinoma of stomach cases were stain by Methylene blue and 99 (85.87%) cases were found heavy dying, and 27 cases were confirmed as early carcinoma of stomach by pathological biopsy. Twenty-four early carcinoma of stomach cases were followed up for 3 years, only 1 case recurred in the 3rd year.Conclusions: Endoscopic dye staining and ligation can improve the diagnostic accuracy. The technique is so easy and safe that it should be applied widely.
