摘要
目的 :监测主动和被动活动下肢及活动的不同时间对股静脉血流速度的影响 ,为前列腺术后卧床期病人选择有效活动方法 ,以预防术后下肢静脉血栓形成。方法 :32例前列腺肥大 ( BPH)术后患者随机分为两组 ,主动活动组 1 6例和被动活动组 1 6例 ,用彩色多普勒血流超声显像仪监测手术前后、活动中与活动后股静脉血流速度。结果 :术后卧床较术前静卧股静脉血流速度明显减慢 ( P<0 .0 5 ) ,主动和被动活动后静卧状态下与术后 5 h静卧状态下股静脉血流速度相比无显著差异 ( P>0 .0 5 )。下肢被动活动 2 min时股静脉血流速度较活动前均明显增加 ( P<0 .0 5 )。主动活动 5 min时较 4min时股静脉血流速度减慢 ,且在活动中心率加快 ,而被动活动自 2 min至 5 min各时点血流速度无变化 ,心率、血压亦无明显影响。结论 :主动活动与被动活动均能使 BPH术后卧床期间股静脉血流速度增快 ,被动活动对心率。
Objective:To monitor the effect of passive and initiative movements of lower extremities at different time on bloodflow velocity in the femoral vein,in order to choose effective exercise methods and prevent phlebothrombosis of leg for patients after prostatectomy during bed rest.Methods:Thirty two postoperative patients with benign prostatic hypertrophy (BPH) were randomly divided into two groups,passive movement group (16 cases) and intiative one (16 cases).Bloodflow velocity in the femoral vein was determined by color Doppler ultrasonic velocimeter before and after operation or movement.Results:During bed rest,the velocity in the femoral vein after operation was slower than that before operation ( P <0 05).After two minites' passive movement of lower extremity,there was an obvious increase of the velocity in the femoral vein compared with that before passive movement ( P <0 05),and so after initiative movement.Conclusion:Both passive and initiative movements can increase the speed in the femoral vein during bed rest of postprostatectomy.Passive movement has no effects on heart rate,blood pressure,and active bleeding at the surface of wound.
