钙粘附素及连环素表达与乳腺癌转移及预后的关系

Reduced Expression of E-cadherin and α-,β-catenins in Human Breast Carcinoma with Relation to Metastasis and Prognosis

目的:了解乳腺癌E-钙粘附素、连环素-α,-β的表达改变与转移及预后的关系。方法:采用免疫组织化学染色、病理组织学,对105例乳腺癌患者术后定期随访,统计学分析。结果:105例乳腺癌三种粘附分子组化染色减弱和消失频率为E-钙粘附素56%,连环素-α64%,连环素-β43%,与正常及非典型增生组差别明显,P值均小于0.001。单因素分析未能找到E-钙粘附素与转移及预后的联系,但证明连环素-α和-β表达下降分别与癌分化程度及与转移、预后相关,P<0.05和P<0.01。三种粘附分子中E-钙粘附素与连环素-α的表达有平行关系,双阳性组的转移率明显低于一种蛋白表达下降组,P<0.05。COX多因素分析三种粘附蛋白表达均未能成为独立影响乳腺癌预后的因素。结论:E-钙粘附素、连环素-α,-β表达的免疫组化反应在乳腺癌呈下降的总趋势,E-钙粘附素与连环素-α的表达有平行关系,双阳性者转移率明显低。在三者之中连环素-β表达下降与癌转移及病程进展有明显相关性,预后相对差。在考虑用细胞间粘附体系为判断乳腺癌预后指标时必须对该三种分子作综合检测。

Objective:To inves ti gate changes of expression of E-cadherin/catenins in breast carcinoma with re lation to metastasis and prognosis.Methods:105archival primary mam-mary ca rcinoimas were immunohistochemically studied using antibodies against E-cadher in,-catenin and-catenin.Histopathology.Post-operation case fellow-up .Statistic procedures.Re-sults:Re duced or absent immunoreactivity for E-cadherin(56%)?-catenin(43%)in breast carci nomas was observed which was different from normal and proliferative group(P= 0.001).As a single factor,reduction of E-cadherin was failed to correl ate with metastasis,however,re duction of-catenin were corel ated with grading and metastais/progno-sis with P value<0.05and<0.0 1respectively.Frequency of comparable results of E-cadherin and-catenin in a same sample was high.Taking E-cadherin and-catenin as combined fac-tor ,a significant association was found between both positive immunoreactivity an d abscence of the metastases(P<0.05).COX multiple factor analysis result s failed to include E-cadherin or-duction of expression of E-cadherin,-catenin and-catenin was a common tendency in breast carcino mas investigated.There were comparable changes between E-cadherin and-caten in.Double positive group showed significant ly lower metastasis.Among th e three, only-tive prognostic parameter,combine d measurement of E-cadherin,-catenin and-catenin should be con sidered .
