CT引导下鼻咽癌放疗后咽旁间隙经皮穿刺切割活组织检查的临床应用

Clinical application of CT-guided percutaneous core cutting-needle biopsy in the recurrence of nasopharyngeal carcinoma at the parapharyngeal space after radiotherapy

目的 :探讨CT引导下鼻咽癌放射治疗后咽旁间隙经皮穿刺切割活组织检查的临床应用 .方法 :1997 0 8/ 2 0 0 2 0 7,32例行CT、MRI及18FDG PET检查后不能确定鼻咽癌放疗后咽旁间隙是否复发或纤维化的患者 ,采用CT引导下经皮穿刺切割活组织检查 ,观察穿刺活检的并发症及病理诊断与临床随访的符合率 .结果 :CT引导下咽旁间隙穿刺切割活组织检查病理诊断与临床随访的符合率为 10 0 %,其主要并发症为穿刺部位疼痛 ,无血管、神经损伤等并发症的发生 .结论 :CT引导下鼻咽癌放疗后咽旁间隙穿刺切割活组织检查是安全。

AIM: To explore clinical application of CT guided percutaneous core cutting needle biopsy (PCNB) in the recurrence of nasopharyngeal carcinoma at the parapharyngeal space after radiotherapy. METHODS: From August, 1997 to July, 2002, 32 cases, in whom the recurrence or fibrosis of nasopharyngeal carcinoma at the parapharyngeal space after radiotherapy could not be diagnosed by CT, MRT or 18 FDG PET examinations, were given CT guided PCNB to observe the complication of PCNB and pathologic diagnosis coincidence rate of the clinical follow up. RESULTS: Coincidence rate of pathologic diagnosis and clinical follow up was 100%. The complication of CT guided PCNB was ache at the point of puncture without any injury of blood vessel and nerves. CONCLUSION: CT guided PCNB in the recurrence of nasopharyngeal carcinoma at the parapharyngeal space after radiotherapy is feasible, successful, and safe.
