摘要 目的 :通过分析 1 8例人工耳蜗植入术后应用神经反应遥测 (neuralresponsetelemetry ,NRT)技术辅助调机 ,确定行为反应阈值 (T level,T级 )和最大舒适级 (C level,C级 ) ,试图为幼儿、合并其他残疾或不能配合调试的植入者估计行为反应T ,C级提供依据 .方法 :受试对象为使用Nucle usCI2 4M或NucleusCI2 4RCS型多导人工耳蜗系统的 1 8例患者 ,测试软件为NRT3.0版本 ,硬件包括计算机、IF5卡、调试控制台 (processorcontrolinterface,PCI)及多导人工耳蜗系统 .NRT阈值的测试采用单极刺激方式分别测试每一例患者的奇数电极 .T ,C级行为测试均于同日进行 .结果 :1 8例患者的 1 80个电极的波形检出率为 85 % ,所有电极的NRT阈值均大于T级 ,NRT阈值超过C级的电极占 2 2 % ,NRT平均阈值介于T级和C级的均值之间 .结论 :NRT技术可以客观反映人工耳蜗植入手术的有效性 ,而且为不能配合调试的患者提供了估计T级和C级的参考值 。 AIM: To provide an estimating method for T level and C level by using NRT techniques for those patients who are either too young or have other disabilities. METHODS: 18 patients implanted with the Nucleus CI24M or Nucleus CI24RCS multiple cochlear implant system were included in this study. The application software was NRT 3.0 version and the hardware was made up of computer, IF5 card, PCI (processor control interface) and Nucleus multiple cochlear implant system. Monopolar stimulation mode was used in NRT threshold measurement on the odd number electrodes. The T level and C level were obtained at the same visit. RESULTS: NRT thresholds were obtained in 85% tested electrodes, which were all higher than T level. 30% NRT thresholds were even higher than C level. The mean NRT thresholds lay between the mean value of T level and C level. CONCLUSION: NRT techniques practically reflect the effectiveness of cochlear implant surgery. Further more, it provides an objective reference index of T level and C level for those patients who are unable to cooperate during the mapping after surgery.
