CT导引肺内小结节(≤2cm)穿刺活检

CT-guided needle biopsy of small intrapulmonary nodules (less than 2 cm in diameter)

[目的 ]探讨CT导引肺内小结节 (≤ 2cm )穿刺活检的临床应用价值。 [方法 ]31例 ,在CT导引下应用自动切割式活检针取材。所有病例均同时行组织学和细胞学检查。 [结果 ]穿刺取材成功率为 90 .3% (2 8/ 31) ,恶性结节组织学准确率为 84 .2 % (16 / 19) ,细胞学准确率为 89.5 % (17/ 19) ,二者结合准确率 89.5 % (17/ 19)。良性结节分别为 6 6 .7% (6 / 9)和 77.8% (7/ 9) ,二者结合准确率88.9% (8/ 9)。气胸发生率为 10 / 31(32 .6 % )。 [结论 ]CT导引肺内小结节穿刺活检安全可行。

To explore the clinical value of CT-guided needle biopsy of the small intrapulmonary nodules(less than 2 cm in diameter). CT-guided percutaneous biopsy were performed in 31 cases with automated cutting needle. Bioptic samples were examined in histology and cytology simultaneously. The accuracy of puncture was 90.3 % (28/3); Diagnostic accuracy of malignant tumor was 84.2 % (16/19) in cytologic study and 89.5 %(17/19) in histologic examination. The overall diagnostic accuracy was 89.5 % (17/19). Diagnostic accuracy of benign nodules was 66.7 % (6/9) in cytologic study and 77.8 %(7/9) in histologic examination. The overall diagnostic accuracy was 88.9 % (8/9). The incidence of pneumothorax was 32.6 % (10/31). [Conclusion] CT-guided needle biopsy of the small intrapulmonary nodules is a safe and effective procedure..
