双极松解术矫治4岁以上儿童先天性肌性斜颈

Surgical correction of congenital muscular torticollis in children over 4 years old with bipolar release

目的:探讨双极松解术矫治4岁以上先天性肌性斜颈患儿的疗效。方法:26例(男17例,女9例)4～14岁的患儿接受了双极松解术,术后应用颈围领1个月,同时接受康复功能锻炼3个月。术前术后参照先天性肌性斜颈评分系统评定各项功能指标,包括颈部活动、头部倾斜和面部不对称。结果:22例(85%)完全纠正了头部倾斜,23例(88%)获得了正常的颈部活动。20例(77%)仅遗留轻度面部发育不对称,仍有4例和2例分别存在中度和重度的面部不对称。结论:4岁以上儿童先天性肌性斜颈可通过双极松解术得到满意的结果,尤其在改善颈部活动和头部倾斜方面效果显著。

AIM:To explore the effect of bipolar release on congenital muscular torticoll is in patients over 4 years old. METHODS:Twenty- six patients(17 males,9 females) aged from 4 to 14 years old were performed bipolar release,followed by one month of neck collar applying an d 3 months of physiotherapy.Preoperative and postoperative functional indices we re evaluated with congenital torticollis scoring system, including neck movement ,head tilt,facial asymmetry. RESULTS:The average follow- up period was 30 months,22(85% )patients had no head tilt,23(88% ) regained full range of motion of the neck,20 (77% ) had mi ld facial asymmetry,4 patients had moderate and 2 patients retained severe facia l asymmetry. CONCLUSION:Congenital muscular torticollis patients,aged over 4 years old can benefit a lot from bipolar release, especially in head tilt and range of neck m ovement.
