Non-operative Treatment of Peptic Ulcer with Acute Perforation:A Report of 25 Cases

目的 探讨非手术治疗消化性溃疡急性穿孔的适应证和治疗方法。方法 回顾分析我们非手术方法治疗 2 5例消化性溃疡穿孔病人的临床资料。结果 除 1例并发肠间隙脓肿外余非手术治疗效果良好 ,平均住院日为 10 .5天 ,此后均经正规内科药物治疗和胃镜检查 ,14例获访中 2例再发溃疡出血而行胃大部切除术。结论 年轻患者消化性溃疡空腹早期穿孔 ,可试行非手术治疗。症状不缓解、全身炎症反应加重者应及时手术治疗。

Objectives To explore method and indication of non-operative treatment for the peptic ulcer with acute perforation. Methods The clinical data of patients diagnosed as peptic ulcer with acute perforation were analyzed retrospectively. Results In these patients, curative effect of non-operative treatment is good, except one patient who complicated with abscess. Average days in hospital are 10.5 days. Two of them were treated with gastrectomy, because of bleeding caused by relapse of peptic ulcer. Conclusions Non-operative treatment of the peptic ulcer with acute perforation, as a selective method, is adapted to empty and forepart acute perforation of younger. During non-operative treatment, if inflammatory response became worse, operative treatment should be performed immediately.
