基层胸痛中心建设对急性心肌梗死救治的意义

Significance on the treatment of acute myocardial infarction with the construction of basic chest pain center

目的探讨基层胸痛中心的建立对急性心肌梗死救治的意义,为临床疾病的治疗提供参考。方法选取东莞市常平医院2016年7月-2018年7月收治的108例急性心肌梗死患者为研究对象,随机分为观察组和对照组各54例,其中对照组给予传统临床路径进行救治,观察组给予基层胸痛中心绿色通道进行救治,观察两组患者的救治效率、冠脉造影及PCI手术情况、出院后生活质量SF-36评分情况。结果观察组入院至球囊扩张时间、住院时间及院内病死率均低于对照组,差异有统计学意义(P<0.05);两组冠状动脉造影诊断正常率及PCI手术率差异无统计学意义(P>0.05);观察组出院后1个月及3个月生活质量评分均高于对照组,差异有统计学意义(P<0.05)。结论基层胸痛中心的建立可有效提高急性心肌梗死患者救治效率,降低病死率,改善患者生活质量,值得临床推广使用。

Objective To explore the significance of the establishment of primary chest pain center in the treatment of acute myocardial infarction,and to provide reference for the treatment of clinical diseases.Methods 108 patients with acute myocardial infarction admitted to Changping Hospital of Dongguan from July 2016 to July 2018 were randomly divided into observation group and control group with 54 cases in each group.The control group was treated by traditional clinical pathway of acute myocardial infarction,and the observation group was treated by green channel of basic chest pain center.Rescue efficiency,coronary angiography and PCI,SF-36 scores of quality of life after discharge were analyzed.Results The time from admission to balloon dilatation,hospitalization time and hospital mortality in the observation group were significantly lower than those in the control group(P<0.05);there was no significant difference between the two groups in the normal diagnostic rate of coronary angiography and PCI operation rate(P>0.05);the quality of life scores of one month and three months after discharge of the observation group were significantly higher than those of the control group(P<0.05).Conclusion The establishment of basic chest pain center can significantly improve the efficiency of treatment of acute myocardial infarction,reduce mortality,improve the quality of life of patients,and it's worthy of clinical application.
