超声弹性成像对甲状腺良恶性结节的诊断价值

The diagnostic value of ultrasound elastography in benign and malignant thyroid nodules

摘要 目的探讨超声弹性成像与二维超声对甲状腺良恶性结节的诊断价值,比较两者在甲状腺良恶性结节超声诊断中的优势。方法选取2015年10月-2018年11月在天津市宝坻区人民医院就诊的甲状腺结节患者135例,均行二维超声与超声弹性成像检查。对恶性结节的相关指标进行观察,分析两者的敏感性、特异性、阳性预测值、阴性预测值以及准确度。结果常规二维超声检查对甲状腺恶性结节的灵敏性60.6%、特异性70.6%、阳性预测值40.0%、阴性预测值84.7%、准确度68.1%;超声弹性成像检查对甲状腺恶性结节的灵敏性87.9%、特异性90.2%、阳性预测值74.4%、阴性预测值95.8%、准确度88.9%,差异有统计学意义(P<0.05)。结论与常规二维超声相比,超声弹性成像技术诊断甲状腺良恶性结节的准确性明显提高。 Objective To evaluate the diagnostic value of ultrasound elastography and two-dimensional ultrasound in benign and malignant thyroid nodules,and to compare the advantages of the two method in ultrasound diagnosis of benign and malignant thyroid nodules.Methods 135 patients with thyroid nodules were selected from October 2015 to November 2018 in Tianjin Baodi District People’s Hospital.All patients underwent two-dimensional ultrasound and ultrasound elastography.The related indexes of malignant nodules were observed,such as sensitivity,specificity,positive predictive value,negative predictive value,and accuracy.Results The sensitivity of the two-dimensional ultrasonography to malignant thyroid nodules was 60.6%,the specificity was 70.6%,the positive predictive value was 40.0%,the negative predictive value was 84.7%,and the accuracy was 68.1%.The sensitivity of ultrasound elastography to malignant thyroid nodules was 87.9%,specificity of 90.2%,positive predictive value of 74.4%,negative predictive value of 95.8%and accuracy of 88.9%.The differences were statistically significant(P<0.05).Conclusion The diagnostic accuracy of benign and malignant thyroid nodules was significantly improved by ultrasound elastography compared with conventional two-dimensional ultrasound.
