中西医结合治疗阑尾周围脓肿82例疗效观察

Curative observation on 82 cases with periappendiceal abscess treated by combination of traditional Chinese medicine and Western medicine

目的探讨中西医结合治疗阑尾周围脓肿的临床疗效。方法回顾性分析2016年6月1日-2018年6月1日天津市东丽区东丽医院收治的82例阑尾周围脓肿患者的临床资料,所有患者在抗生素治疗的基础上加用中医汤剂口服或灌肠配合治疗,观察治疗效果。结果成功行非手术治疗80例,另2例因阑尾脓肿腔较大,腹腔炎症重,行彩超引导下脓肿穿刺置管引流术,术后给予口服中药汤剂治疗。所有患者均于加用中药汤剂后24 h内排气、排便,肠梗阻症状明显好转,患者平均住院时间(10.0±2.5)d,均康复出院。结论中西医结合治疗阑尾周围脓肿,可有效缓解盆腔炎症,促进患者排气、排便,治疗效果满意,值得临床应用推广。

Objective To explore the clinical efficacy of combination of traditional Chinese medicine and Western medicine in the treatment of periappendiceal abscess.Methods Clinical data of 82 cases with periappendiceal abscess admitted to Dongli Hospital of Tianjin Dongli District from June 1,2016 to June 1,2018 were retrospectively analyzed.All patients were treated with oral or enema decoction of traditional Chinese medicine on the basis of antibiotic treatment,and the therapeutic effect was observed.Results 80 cases were processed non-operation treatment successfully.For the other 2 cases,because of the large appendix abscess cavity and severe abdominal inflammation,color Doppler ultrasound-guided abscess puncture and catheter drainage was performed,oral Chinese herbal decoction was given to treat after operation.All patients exhausted and defecated within 24 hours after the addition of traditional Chinese medicine decoction,the symptoms of intestinal obstruction improved significantly.The average hospitalization time of the patients was(10.0+2.5)days.All patients recovered and discharged from hospital.Conclusion The treatment of periappendiceal abscess with combination of traditional Chinese and Western medicine can effectively alleviate pelvic inflammation,promote patients’exhaust and defecation,and the therapeutic effect is satisfactory.It is worthy of clinical application and promotion.
