TCCS与CDU联合诊断缺血性脑血管病的临床解析

Clinical analysis of combined diagnosis of ischemic cerebrovascular disease by TCCS and CDU

目的探究经颅双功能彩色多普勒超声(TCCS)与颈部血管超声(CDU)联合检查在急性缺血性脑血管病诊断中的临床应用价值。方法随机抽取2016年6月-2018年8月天津市东丽区东丽医院接受磁共振血管成像(MRA)及磁共振成像(MRI)检查确诊为脑血管病的患者186例,将MRA检查结果作为金标准,评价TCCS与CDU联合检查对颅内外血管狭窄患者诊断准确率,分析颅内血管狭窄伴有交通支是否开放对脑实质损害大小的相关性。结果 TCCS与CDU联合检查对颅内血管狭窄、颈部血管狭窄、斑块情况检测率与MRA检查结果比较,差异无统计学意义(P>0.05)。颅内血管狭窄伴有交通支开放组对脑实质损害明显低于未开放组,差异有统计学意义(P<0.01)。结论对采用TCCS与CDU联合检查对缺血性脑血管病患者更利于提高临床诊断准确率、明确患者病变程度,对临床诊断及治疗方案的确定可提供重要的参考价值。

Objective To explore the clinical application value of transcranial double-functional color Dopple r ultrasound(TCCS)and neck vascular ultrasound(CDU)in the diagnosis of acute ischemic cere-brovascular disease.Methods One hundred and eighty-six patients with cerebrovascular diseases diagnosed by magnetic resonance angiography(MRA)and magnetic resonance imaging(MRI)from June 2016 to August2018 in Dongli Hospital,Dongli District,Tianjin were randomly selected.The MRA results were used as the gold standard.To evaluate the diagnostic accuracy of TCCS combined with CDU in patients with intracranial and extracranial vascular stenosis,and to analyze the correlation between brain parenchymal damage and intracranial vascular stenosis accompanied by communicating branch opening.Results There were no significant differences in the detection rates of intracranial vascular stenosis,cervical vascular stenosis and plaque between TCCS and CDU combined examination and MRA examination(P>0.05).Brain parenchymal damage in the intracranial vascular stenosis with open communicating branch group was significantly lower than that in the unopen group,the difference was statistically significant(P<0.01).Conclusion The combined examination of TCCS and CDU was more helpful to improve the accuracy of clinical diagnosis and clarify the degree of pathological changes in patients with ischemic cerebrovascular disease,could provide important reference value for clinical diagnosis and treatment.
