腹腔镜手术治疗卵巢畸胎瘤72例临床分析

Clinical analysis on 72 patients with ovarian teratoma treated by laparoscopic surgery

目的:探讨腹腔镜手术治疗卵巢畸胎瘤的临床效果。方法:回顾性分析成熟卵巢畸胎瘤患者144例,分为治疗组与对照组各72例,对照组采用常规开腹手术治疗,治疗组采用腹腔镜下手术治疗。结果:治疗组的术中出血量、术后镇痛药使用率与术后住院时间都明显少于对照组(P<0.05)。术后治疗组的切口渗液、切口脂肪液化、血肿、切口感染等并发症发生率明显少于对照组(P<0.05)。术后1个月两组均出现E2水平降低,FSH及LH水平升高,与术前对比差异有统计学意义(P<0.05),上述指标在术前与术后1个月组间比较都无统计学差异。结论:腹腔镜手术治疗卵巢畸胎瘤具有创伤少、恢复快、并发症少等优点,对于患者的卵巢储备功能的影响无放大作用,值得推广应用。

Objective: To explore the clinical effect of laparoscopic surgery in treatment of ovarian teratoma. Methods: A total of144 patients with mature ovarian teratoma were analyzed retrospectively and divided into treatment group and control group,72 patients in each group; the patients in control group were treated with conventional laparotomy,while the patients in treatment group were treated with laparoscopic surgery. Results: The amount of intraoperative blood loss,postoperative analgesic utilization rate and postoperative hospitalization time in treatment group were statistically significantly lower than those in control group( P < 0. 05). The incidence rates of postoperative incisional drainage,incisional fat liquefaction,hematoma and infection of incisional wound in treatment group were statistically significantly lower than those in control group( P < 0. 05). At one month after surgery,estradiol levels in the two groups decreased,the levels of FSH and LH in the two groups increased,compared with before surgery,there were statistically significant differences( P < 0. 05),there was no statistically significant difference in the above- mentioned indexes between before surgery and at one month after surgery. Conclusion: Laparoscopic surgery has the advantages of less trauma,quicker recovery and fewer complications in treatment of ovarian teratoma,and it has no amplification ovarian reserve function of the patients,which is worth popularizing and applying.
