联合检测妊娠早中期孕妇血清β-hCG、Smac及sHLA-G对预测子痫前期的意义

Significance of combined detection of serum β-hCG,Smac,and sHLA-G in predicting preeclampsia in early and middle pregnancy

目的探讨联合检测妊娠早中期孕妇血清β-人绒毛膜促性腺激素(β-hCG)、Smac及可溶性人类白细胞抗原G(sHLA-G)对预测子痫前期的意义,为早期预测子痫前期的临床手段提供参考。方法选取80例子痫前期孕妇为观察组和40例正常孕妇为对照组,检测两组孕妇血清β-hCG、Smac及sHLA-G水平,并计算其对子痫前期的诊断价值。结果早孕期子痫前期孕妇血清中β-hCG和Smac水平显著高于对照组孕妇,sHLA-G水平显著低于对照组孕妇,差异均有统计学意义(P<0.05)。中孕期子痫前期孕妇血清中β-hCG和Smac水平显著高于对照组孕妇,sHLA-G水平显著低于对照组孕妇,差异均有统计学意义(P<0.05)。联合检测可显著提高子痫前期诊断的敏感度(88.64%)。β-hCG、Smac及sHLA-G单独诊断的AUC分别为0.7366、0.8181和0.7916,三者联合诊断的AUC为0.8753,联合诊断与单独诊断的AUC差异有统计学意义(P<0.05)。结论早、中期孕妇血清β-hCG、Smac及sHLA-G水平检测对子痫早期具有诊断价值。

Objective To explore the significance of combined detection of serum β-hCG,Smac,and sHLA-G in predicting preeclampsia in early and middle pregnancy,provide a reference for early prediction of preeclampsia. Methods Eighty pregnant women with preeclampsia were selected as observation group,forty normal pregnant women were selected as control group. The levels of serum β-hCG,Smac,and sHLA-G in the two groups were detected. The diagnostic value of combined detection for preeclampsia was calculated.Results The levels of serum β-h CG and Smac in early pregnancy preeclampsia group were statistically significantly higher than those in control group( P<0. 05). The level of serum sHLA-G in early pregnancy preeclampsia group was statistically significantly lower than that in control group( P<0. 05). The levels of serum β-hCG and Smac in middle pregnancy preeclampsia group were statistically significantly higher than those in control group( P<0. 05). The level of serum sHLA-G in middle pregnancy preeclampsia group was statistically significantly lower than that in control group( P < 0. 05). Combined detection could significantly improve diagnostic sensitivity of preeclampsia( 88. 64%). The values of AUC of single diagnosis of β-h CG,Smac,and sHLA-G were 0. 736 6,0. 818 1,and 0. 791 6,respectively.The value of AUC of combined diagnosis was 0. 875 3. There was statistically significant difference in the value of AUC between combined diagnosis and single diagnosis( P<0. 05). Conclusion Combined detection of serum β-hCG,Smac,and sHLA-G has diagnostic value for preeclampsia in early and middle pregnancy.
