商洛、安康、汉中三市女性宫颈人乳头瘤病毒感染基因型别的对比研究 被引量:1

A comparative study on female cervical HPV infection genotypes in Shangluo,Ankang,and Hanzhong

摘要 目的对陕西省陕南地区的商洛市、安康市和汉中市妇女人群人乳头状瘤病毒(HPV)感染及亚型分布特征进行对比研究,为宫颈癌筛查工作提供数据参考。方法采用PCR-反向斑点杂交技术对来自商洛市1 901例,安康市2 703例,汉中市3 058例宫颈脱落细胞标本进行HPV基因分型检测。结果商洛市妇女人群总感染率为60. 07%;高危感染率为51. 18%。低危感染率为26. 14%;安康市妇女人群HPV感染率为21. 94%,高危HPV感染率为19. 09%,低危HPV感染率为4. 48%;汉中市妇女人群感染率为42. 67%,高危感染率为40. 88%,低危感染率为4. 35%;商洛市感染率和高危感染率最高,其次为汉中市,安康市最低,差异有统计学意义(P<0. 05)。商洛市的低危感染率高于安康市和汉中市(P<0. 05);安康市和汉中市的低危感染率差异无统计学意义(P>0. 05)。商洛市各年龄段的感染率、高危感染率最高,其次为汉中市和安康市,除51~60岁和≥61岁年龄段商洛市和汉中市高危感染率比较差异无统计学意义外,其余各组差异均有统计学意义(均P<0. 05)。商洛市各年龄段的低危感染率高于汉中市和安康市(P<0. 05);安康市和汉中市低危感染率比较,除≥61岁年龄组安康市高于汉中市外,其余各年龄段差异无统计学意义(P>0. 05)。三城市高危感染均以16型最为常见。商洛市其次是58,66,33,18,52型;安康市其次为52,58,53,39,33型,汉中其次为58,52,53,39,33型。低危感染商洛为6,43,42,11,81型;安康为81,11,6,42,43型。汉中市为6,43,11,81,42型。三城市的单一感染均最为常见,其次为二重感染,三重及三重以上感染率较低。安康市单一感染率低于商洛市和汉中市(P<0. 05)。二重感染和三重及三重以上感染率三城市比较商洛市较高,其次为汉中市和安康市(P<0. 05)。单一低危感染率、二重低危感染率汉中市低于商洛市和安康市(P<0. 05);单一高危感染率汉中市较高,其次是商洛市和安康市(P<0. 05);二重高危感染率比较差异无统计学意义(P>0. 05);三重及三重以上高危感染率商洛市较高,其次为汉中市和安康市(P<0. 05),二重混合感染率和三重及三重以上混合感染率商洛高于安康和汉中(P<0. 05)。结论商洛市、安康市和汉中市妇女HPV感染状况各有其特点,研究各地HPV感染状况对宫颈癌的防治具有重要意义。 To compare and research human papillomavirus(HPV)infection status and distribution of HPV genotypes among the women in Shangluo,Ankang,and Hanzhong,provide a reference for cervical cancer screening.Methods HPV genotyping was performed in cervical exfoliative cell specimens of 1 901 women from Shangluo,2 703 women from Ankang,and 3 058 women from Hanzhong by PCR reverse spot hybridization technology.Results In Shangluo,the total HPV infection rate was 60.07%,high-risk HPV infection rate and low-risk HPV infection rate were 51.18%and 26.14%,respectively.In Ankang,the total HPV infection rate was 21.94%,high-risk HPV infection rate and low-risk HPV infection rate were 19.09%and 4.48%,respectively.In Hanzhong,the total HPV infection rate was 42.67%,high-risk HPV infection rate and low-risk HPV infection rate were 40.88%and 4.35%,respectively.The total HPV infection rate and high-risk HPV infection rate in Shangluo were the highest,followed by Hanzhong,which were the lowest in Ankang,there was statistically significant difference(P<0.05).The low-risk HPV infection rate in Shangluo was statistically significantly lower than those in Ankang and Hanzhong(P<0.05).There was no statistically significant difference in the low-risk HPV infection rate between Ankang and Hanzhong(P>0.05).The total HPV infection rate and high-risk HPV infection rate in Shangluo were the highest in each age group,followed by Hanzhong and Ankang.There was no statistically significant difference in high-risk HPV infection rates of women in 51-60-year-old group and≥61-year-old group between Shangluo and Hanzhong,while there were statistically significant differences in high-risk HPV infection rates of women in other age groups(P<0.05).The low-risk HPV infection rate in each age group in Shangluo was statistically significantly higher than those in Hanzhong and Ankang(P<0.05).The low-risk HPV infection rate of women in≥61-yearold group in Ankang was statistically significantly higher than that in Hanzhong,there was no statistically significant difference in low-risk HPV infection rate of women in other age groups between Ankang and Hanzhong(P>0.05).HPV 16 was the most common HPV genotype in the three cities,followed by HPV 58,HPV 66,HPV 33,HPV 18,HPV 52 in Shangluo,HPV 52,HPV 58,HPV 53,HPV 39,HPV 33 in Ankang,HPV 58,HPV 52,HPV 53,HPV 39,HPV 33 in Hanzhong.The common low-risk HPV genotypes in Shangluo were HPV 6,HPV 43,HPV 42,HPV 11,HPV 81.The common low-risk HPV genotypes in Ankang were HPV 81,HPV 11,HPV 6,HPV 42,HPV 43.The common low-risk HPV genotypes in Hanzhong were HPV 6,HPV 43,HPV 11,HPV 81,HPV 42.Single HPV infection was the most common type in the three cities,followed by double HPV infection,triple HPV infection or above were not common.The single HPV infection rate in Ankang was statistically significantly lower than those in Shangluo and Hanzhong(P<0.05).The double HPV infection rate,triple HPV infection rate or above in Shangluo were the highest,followed by Hanzhong and Ankang(P<0.05).The single lowrisk HPV infection rate and double low-risk HPV infection rate in Hanzhong were statistically significantly lower than those in Shangluo and Ankang(P<0.05).The single high-risk HPV infection rate in Hanzhong was the highest,followed by Shangluo and Ankang(P<0.05).There was no statistically significant difference in double high-risk HPV infection rate among the three cities(P>0.05).The triple highrisk HPV infection or above in Shangluo were the highest,followed by Hanzhong and Ankang(P<0.05).The double mixed HPV infection rate,triple mixed HPV infection or above in Shangluo were statistically significantly higher than those in Ankang and Hanzhong(P<0.05).Conclusion HPV infection in Shangluo,Ankang,and Hanzhong has different characteristics.Researching HPV infection statuses in different regions has improtant significance in prevention and treatment of cervical cancer.
