穴位针刺疗法联合中药敷贴对寒凝血瘀型原发性痛经症状改善及血清学指标影响研究

Study on the effect of acupoint acupuncture combined with traditional Chinese medicine plaster on improvement of symptoms and serum markers of patients with primary dysmenorrhea of cold congealing and blood stasis type

目的探讨穴位针刺疗法联合中药敷贴对寒凝血瘀型原发性痛经症状改善及血清学指标影响,为有效改善痛经各项症状、调节血清指标水平提供参考。方法选择2016年10月-2018年6月于温州医科大学附属第一医院诊治的80例寒凝血瘀型原发性痛经患者随机分为观察组和对照组各40例,观察组患者给予穴位针刺疗法联合中药敷贴治疗,对照组给予单独中药敷贴治疗,比较两组患者的治疗效果、VAS疼痛评分、中医证候积分、痛经症状改善情况以及血清学指标的变化。结果经3个月治疗后,观察组总有效率95. 00%显著高于对照组的77. 50%(P<0. 05);两组患者的VAS评分、中医证候积分、痛经持续时间、痛经症状严重程度评分较治疗前均明显下降,观察组较对照组下降更为显著(P<0. 05);两组患者PGE2水平较治疗前均明显上升,PGE2α水平较治疗前均明显下降,观察组较对照组上升或下降更为显著(P<0. 05)。结论穴位针刺疗法联合中药敷贴治疗寒凝血瘀型原发性痛经疗效显著,可有效改善痛经各项症状,调节血清指标PGE2、PGE2α水平,建议在临床推广应用。

Objective To explore the effect of acupoint acupuncture combined with traditional Chinese medicine plaster on improvement of symptoms and serum markers of patients with primary dysmenorrhea of cold congealing and blood stasis type,provide a reference for effecrive improvement of dysmenorrheal symptoms and regulation of levels of serum markers.Methods Eighty patients with primary dysmenorrhea of cold congealing and blood stasis type who were treated in the First Affiliated Hospital of Wenzhou Medical University from October 2016 to June 2018 were selected and randomly divided into observation group(40 cases)and control group(40 cases).The patients in observation group were treated with acupoint acupuncture combined with traditional Chinese medicine plaster,and the patients in control group were treated with traditional Chinese medicine plaster.The curative effects,VAS scores,traditional Chinese medicine syndrome scores,improvement degrees of dysmenorrheal symptoms,and changes of serological markers were compared between the two groups.Results After 3 months of treatment,the total effective rate in observation group was 95.00%,which was statistically significantly higher than that in control group(77.50%)(P<0.05).After 3 months of treatment,VAS scores,traditional Chinese medicine syndrome scores,the duration time of dysmenorrheal,and the severity of dysmenorrhea were statistically significantly lower than those before treatment,especially in observation group(P<0.05).After 3 months of treatment,the levels of PGE2 in the two groups were statistically significantly higher than those before treatment,while the levels of PGE2αwere statistically significantly lower than those before treatment,especially in observation group(P<0.05).Conclusion The curative effect of acupoint acupuncture combined with traditional Chinese medicine plaster in treatment of patients with primary dysmenorrhea of cold congealing and blood stasis type is good,the method can effectively improve the symptoms of dysmenorrhea and regulate serum levels of PGE2 and PGE2α.It is recommended to be applied in clinic.
