乙型肝炎病毒感染孕产妇母乳喂养意愿的质性研究

Qualitative study on breastfeeding willingness of pregnant women with HBV infection

目的了解乙型肝炎病毒(HBV)感染孕产妇母乳喂养意愿及其原因并探讨其应对方法。方法使用现象学研究法对16例HBV感染孕产妇进行访谈,使用Co Laizzi7步法整理分析资料。结果分析显示HBV感染孕产妇普遍担心母乳喂养会增加孩子感染HBV的风险;对HBV传播途径以及乙型肝炎母婴阻断相关知识较缺乏;医生、家人尤其是丈夫的劝导对单纯HBV感染孕产妇是否母乳喂养的影响较大。结论规范实施HBV感染孕产妇母婴阻断的各项措施意义重大,构建HBV感染孕产妇母乳喂养支持体系非常必要;应规范开展产前母乳喂养咨询门诊,提高HBV感染孕产妇及家庭对母乳喂养的认知。

Objective To understand the breastfeeding willingness and causes of pregnant women with hepatitis B virus(HBV)infection,explore the coping strategies.Methods Sixteen pregnant women with HBV infection were interviewed by phenomenological research method,and Co Laizzi7 footwork was used to collate and analyze the data.Results HBV-infected pregnant women were generally worried that breastfeeding might increase the risk of HBV infection,the pregnant women lacked the knowledge about HBV transmission routes and HBV block between mother and infant,the persuasion of doctors,family members,especially husbands,had a great impact on breastfeeding of simple HBV-infected pregnant women.Conclusion It is of great significance to standardize the implementation of HBV measures for mother to child transmission of HBV-infected pregnant women,it is very necessary to build HBV support system for breastfeeding of pregnant women,prenatal breastfeeding consultation outpatient service should be standardized to improve the cognition of HBV-infected pregnant women and families on breastfeeding.
