基于住院病案首页乳腺癌化疗患者数据分析

Data Analysis of Breast Cancer Chemotherapy Patients Based on the Front Pages of Inpatient Medical Record

目的对某三甲医院近几年乳腺癌化疗患者的病案首页数据进行挖掘分析,了解其时间成本和费用成本,为疾病负担分析提供线索。方法检索2014年1月1日-2017年12月31日住院病案首页数据中ICD编码同时包括Z51和C50的住院患者,共4544人次,474人,分析时间和费用指标。结果平均每人化疗9.6次,多次化疗的间隔平均18.7天,平均住院日为2.68天,平均每人花费53 466.54元,次均住院花费5577.28元,不同分型采用的治疗方式主要为口服化疗药物、静脉注射化疗药物与分子靶向治疗,且费用不尽相同。结论乳腺癌化疗要花费大量时间成本和费用成本,每人平均总成本将超过15万,间接成本要多于直接成本,不同的影响因素也会影响费用,分型严重、晚期、未纳入医保的患者疾病负担大。

Objective To explore and analyze the front pages data of breast cancer chemotherapy patients in a third grade a hospital in recent years, understand the time cost and hospitalization cost, and provide clues for disease burden analysis. Methods From January 1, 2014 to December 31, 2017, the ICD code in the front pages of inpatient medical record data included both Z51 and C50 inpatients, a total of 4,544 person-times and 474 people,and the time and cost indicators were analyzed. Results The average chemotherapy was 9.6 times per person, the interval of multiple chemotherapy was 18.7 days, the average hospitalization day was 2.68 days, the average cost per person was 53466.54 yuan, the average hospitalization cost was 5577.28 yuan, the treatment methods used by different types were mainly oral chemotherapy drugs, intravenous chemotherapy drugs and molecular targeted therapy, and the costs were different. Conclusions Chemotherapy for breast cancer would cost a lot of time and cost,the average total cost per person would be more than 150,000 yuan, the indirect cost was more than the direct cost,different influencing factors would also affect the cost, classification of severe, advanced, not included in the medical insurance patients with a large disease burden.
