经鼻持续气道正压通气治疗小儿重症肺炎临床观察 被引量:13

Clinical Observation of Nasal Continuous Positive Airway Pressure in Treating Severe Pneumonia in Children

摘要 目的观察经鼻持续气道正压通气治疗小儿重症肺炎的临床疗效。方法选择某院自2017年6月1日-2018年6月1日收治的160例小儿重症肺炎患儿作为研究对象,将其随机分成对照组和研究组,每组80例,对照组采用鼻氧管治疗,研究组采用经鼻持续气道正压通气治疗,对比两组患儿临床疗效。结果治疗前,两组患儿呼吸、心率及Pa O2、Pa CO2、HCO3-等血气指标对比均无显著差异(P>0.05);治疗后,研究组患儿呼吸(42.7±3.3)次/min、心率(128.3±6.2)次/min及Pa CO2(6.5±1.2)kPa明显低于对照组(54.8±3.5)次/min、(155.4±6.5)次/min、(7.6±1.5)k Pa,研究组患儿Pa O2(7.5±1.6)k Pa、HCO3-(21.9±3.1)mmol/L及治疗的总有效率(98.8%)明显高于对照组(6.4±1.2)k Pa、(17.8±2.5)mmol/L、(77.5%),研究组患儿呼吸困难、发绀、肺部罗音等症状体征持续时间及住院时间(0.9±0.2)d、(14.3±1.5)h、(7.3±1.6)d、(9.8±1.5)d均明显短于对照组(1.8±0.4)d、(29.6±2.3)h、(8.9±2.2)d、(10.2±2.5)d,两组对比差异显著(P<0.05)。结论对小儿重症肺炎患儿采用经鼻持续气道正压通气治疗,不仅能有效改善患儿呼吸困难、气促等症状,同时还能缩短患儿预后周期,因此,其是一种高效的临床治疗方案。 Objective To observe the clinical efficacy of nasal continuous positive airway pressure in the treatment of severe pneumonia in children. Methods 160 children with severe pneumonia who were treated in hospital from June 1, 2017 to June 1, 2018 were selected as study subjects. They were randomly divided into control group and study group, 80 cases in each group. For nasal oxygen therapy, the study group used nasal continuous positive airway pressure ventilation to compare the clinical efficacy of the two groups of children.Results Before treatment, there were no significant differences in respiratory and heart rate and blood gas indexes such as PaO2, Pa CO2 and HCO3-between the two groups(P>0.05). After treatment,the study group had respiratory(42.7±3.3)times/min and heart rate.(128.3±6.2)times/min and PaCO2(6.5±1.2)kPa were significantly lower than the control group(54.8±3.5)times/min,(155.4±6.5)times/min,(7.6±1.5)k Pa,children in the study group PaO2(7.5±1.6)kPa, HCO3-(21.9±3.1)mmol/L and the total effective rate of treatment(98.8%) were significantly higher than those of the control group(6.4± 1.2)k Pa,(17.8±2.5)mmol/L,(77.5%),the study group’s children with dyspnea,cyanosis,lung rales and other symptoms and signs duration and hospital stay(0.9±0.2)d、(14.3±1.5)h、(7.3±1.6)d、(9.8 ±1.5)d was significantly shorter than the control group(1.8±0.4)d,(29.6±2.3)h,(8.9±2.2)d,(10.2±2.5)d, and the difference between the two groups was significant(P<0.05). Conclusions The use of nasal continuous positive airway pressure ventilation in children with severe pneumonia in children not only could effectively improve the symptoms of children with dyspnea and shortness of breath, but also could shorten the period of prognosis of children. Therefore, it was a highly effective method. Clinical treatment plan.
