急性髓系白血病患者住院费用及影响因素

Hospitalization Expenses and Influencing Factors in Patients with Acute Myeloid Leukemia

目的分析急性髓系白血病患者住院费用构成及变化情况,探讨住院费用的影响因素。方法收集2013年1月1日-2017年12月31日人院的急性髓系白血病患者的病例资料,对住院费用进行统计描述,对其影响因素进行单因素分析和多元线性回归分析。结果共纳入病例278例,平均年龄为50. 88±16. 37岁,年均住院费用为5. 58万元,次均住院费用为24126. 08元,日均住院费用为1239. 46元;药费占比最高,为63.96%;单因素与多因素分析结果显示住院天数、是否初诊、年龄和临床分型等因素差异有统计学意义。结论住院费用中药费和血费相对较高;住院天数、是否初诊、年龄和临床分型是影响住院费用的主要因素,适当降低药费和血费,能有效减轻家庭和社会的经济负担。

Objectives To analyze the composition and changes of hospitalization expenses in patients with acute myeloid leukemia(AML)and explore the influencing factors of hospitalization expenses.Methods Basic information and expense information of patients with acute myeloid leukemia admitted from January 1 st,2013 to December 31 st,2017 were collected,and the composition of hospitalization expenses was statistically described.Univariate analysis and multiple linear regression analysis were performed on the influencing factors of hospitalization expenses.Results A total of 278 cases were included in this study,the average age was 50.88±16.37 years old,the average annual hospitalization expense was 55,800 yuan,the average hospitalization expense was 24126.08 yuan,and the average daily hospitalization expense was 1239.46 yuan.The proportion of western medicine expenses is the highest,accounting for 63.96%of the total expenses.Results of univariate and multivariate analyses showed that statistically significant differences existed in age,length of stay,clinical classification,and new diagnosis.Conclusions The expense of hospitalization was relatively high in western medicine and blood products,and the length of stay,new diagnosis,age and clinical classification were the main factors affecting the hospitalization expense of patients with acute myeloid leukemia.Appropriate reduction of medicine and blood products expenses could effectively lighten the economic burden of family and society.
