2007—2013年山东省乡镇卫生院改革发展研究

Study on the Reform and Development of Township Hospitals in Shandong Province during 2007—2013

目的分析2007—2013年山东省乡镇卫生院改革发展情况。方法收集2007—2013年山东省卫生统计年鉴,运用统计描述分析乡镇卫生院人、财、物及医疗服务的发展变化情况。结果 2007—2013年乡镇卫生院人员本科及以上学历者由6.31%增加到9.20%,职称变化不明显,医护比由1∶0.50提高到1∶0.73;资产负债率下降10.24个百分点,财政补助收入所占比例不断提高;除乡镇卫生院数量减少外,卫生设施不断增加,平均每机构床位数由37.88张增加到63.44张;医疗服务数量不断增加,但卫生院诊疗人次、住院人数占全省的比例降低,病床利用率由45.21%增加到55.28%。结论随着政府财政投入力度加大,2007—2013年乡镇卫生院卫生设施得以改善,财政补助比重上升,负债压力缓解,医疗服务数量、效率有所提高;卫生人员的数量、质量有一定程度改善,但仍面临医护比例倒置、素质偏低的问题。

OBJECTIVE To analyze the situation of reform and development of township hospitals in Shandong Province during2007-2013. METHODS Collecting'Health Statistics Yearbook of Shandong Province'and using statistical description to analyze de-velopments and changes of people, financial, material and medical service in township hospitals. RESULTS During 2007-2013, theproportion of undergraduate or higher degree increased from 6.31% to 9.20%,the title did not change significantly, the ratio of doc-tor versus nurse increased from 1:0.50 to 1:0.73; the ratio of asset-liability decreased cumulatively by 10.24%, the proportion of fi-nancial subsidy income to total income improved continuously; compared to 2007, the number of township hospitals reduced, butthe other health facilities increased, the average number of beds per hospital increased from 37.88 to 63.44; the proportion of clinicvisits and hospital admissions to whole province declined, the utilization rate of beds increased from 45.21% to 55.28%. CONCLUSIONS From 2007 to 2013, as the government fiscal investment increased, township hospitals and health facilities were improved, lia-bilities stress relieved, the number of medical services increased; the quantity and quality of health personnel were improved to acertain extent, but the invert of the ratio of doctor and nurse was still serious.
