婴儿42天常规体检髋关节超声筛查结果及相关因素分析

Analysis on the Results of Ultrasound Screening and Related Factors of Routine Examination for 42 days Infants

目的了解六安市金安区婴儿42天常规体检髋关节发育情况和超声筛查发育性髋关节发育不良(developmental dys-plasia of the hip,DDH)的检出率及影响DDH的相关因素,以便于开展早期干预,降低人群DDH的发病率。方法自行设计问卷对来儿童保健门诊进行42天体检的婴儿常规进行Graf超声检查髋关节,填写问卷并记录检查结果并进行分析。结果本次受检婴儿共1 964名。受检共3 928个髋关节,正常髋关节3 772个(96.03%),可疑髋关节150个(3.81%),异常髋关节6个(0.15%)。其中女婴DDH检出率显著高于男婴;左侧髋关节发生DDH的风险高于右侧;臀位及其他胎位的婴儿DDH的检出率高于头位;羊水量少的婴儿DDH的风险高于羊水正常、羊水多组;婴儿双下肢包裹组DDH的发生风险高于未包裹组。结论基层开展DDH筛查,对该疾病的早期发现,合理预防具有重要的意义。

OBJECTIVE To explore the results of ultrasound screening and related factors analysis of routine examination for 42 days in-fants, the detection rate for developmental dysplasia of the hip(DDH), in order to carry out early intervention, reduce the incidence ofDDH in the population. METHODS A self-designed questionnaires were used to examine the infants undergoing 42 days physical ex-amination at the children's health care clinic. The questionnaires were filled and the results were recorded and analyzed. RESULTS The total number of infants under examination was 1964. A total of 3 928 hips, 3 772 of normal hip(96.03%), 150 of suspicious hip(3.81%), and 6 of abnormal hip(0.15%). The detection rate of female DDH was significantly higher than that of the male; the risk ofleft hip DDH was higher than that of the right; the detection of breech and other fetal position DDH was higher than that of the headposition; the risk of DDH with less amniotic fluid was higher than that in normal amniotic fluid group and amniotic fluid group. Therisk of DDH in the double lower limbs enveloped group was higher than that in the non-enveloped group; CONCLUSION DevelopingDDH screening at grass-roots level were of great significance for the early detection of the disease and rational preventive measures.
