显微手术对大脑后动脉巨大动脉瘤的治疗效果观察

Curative effect observation on microsurgery to treat giant aneurysm in posterior cerebral artery

目的探讨分析显微手术对大脑后动脉巨大动脉瘤的治疗效果。方法选取我院(辽宁省大连市大连大学附属中山医院)2008-01—2013-12收治的10例行显微手术的大脑后动脉巨大动脉瘤患者为研究对象,对其临床资料进行回顾性分析。结果10例患者中有5例行动脉瘤夹闭,3例行动脉瘤夹闭并实施瘤体切除术,2例行动脉瘤包裹术。术后9例患者恢复良好,1例仍存在不全象限性偏离。结论对大脑后动脉巨大动脉瘤患者行显微手术时选择合适的手术入路,将动脉瘤及载瘤动脉充分显露,并注意保护神经以及穿支血管,能有效降低术后并发生发生率,改善患者生活质量。

Objective To investigate the curative effect of m icrosurgery in the treatm ent of giant aneurysm in posterior cerebral artery. Methods 10 cases w ith giant aneurysm in posterior cerebral artery adm itted to our hospital for m icrosurgery from January 2008 to D ecem-ber 2013 w ere selected as the research objects,and their clinical data w ere retrospectively analyzed .Results In 10 cases,5 cases had surgi-cal clipping of arterial aneurysm ,3 cases had surgical clipping of arterial aneurysm and the im plem entation of tum or resection,and 2 cases had arterial aneurysm package.A fter m icrosurgery,9 patients got good recovery,but 1 case still had partial quadrant deviation.Conclusion Proper operative approach should be adopted to conduct m icrosurgery in the treatm ent of giant aneurysm in posterior cerebral artery w ith com plete exposure of arterial aneurysm and parental artery,and to protect the nerves and perforator vessels.T his can effectively reduce the incidence rate of postoperative com plications,and im prove the living quality of the patients.
